摘 要:目的评价果糖对创伤后患者血糖、胰岛功能的影响以及临床使用的安全性.方法 采用前瞻、随机、盲法、对照、平行和多中心的研究方法.共120例创伤患者进入本研究.术后1~2 d开始空腹静脉注射5%果糖注射液或5%葡萄糖注射液500 ml,连续3 d.观察用药前和用药中血糖、胰岛素(insulin,INS)和胰岛素C-肽(insulin peptide C,INS C-P)的变化,比较两组之间的差别.同时观察用药前后血尿酸(uric acid,URIC)、乳酸(lactic acid,LACT)和肝肾功能等的变化,以及不良事件.结果 静脉输注5%果糖500 ml对机体血糖影响较小,在输液前后血糖变化值为(0.41±1.72)mmol/L;输注5%葡萄糖血糖明显升高,血糖变化值为(2.51±2.23)mmol/L,与研究组比较差异有显著性(P<0.01).研究组用药中对血胰岛素和胰岛素C肽的影响亦较小;对照组用药中的两指标则明显升高,组间比较有显著性差别(P<0.01).用药后两组患者均未发现血乳酸和尿酸升高,肝肾功能等指标无明显异常变化,未出现不良事件.结论 适量静脉输注果糖对创伤后患者血糖和胰岛功能的影响较小,有较好的安全性.Objective To investigate the effects of fructose on serum glucose, pancreas islet function and evaluate its safety in post-trauma patients. Methods A prospective, double-blind, randomized and controlled, muhicenter clinical study was performed. 120 consented post-trauma patients who met with defined criteria were enrolled into the study. The subjects in the study group were administered with 500 ml of 5% fructose injections intravenously in a series of three days since post-operation 1st or 2nd day. While the subjects in the control group were administered with glucose of the corresponding concentration and volume. The changes of serum glucose, INS and INS C-P, LACT and URIC before and after injection were measured. Adverse events were observed. Results After the injection 5% ml of 5% fructose, the changes of serum glucose were only (0.41±1.72)mmol/L. However, the serum glucose increased remarkably in the control group, which was (2.51±2.23) mmol/L. The difference between the study group and the control group was significant ( P〈0.01). In the study group, the intravenous injection of 5% fructose had little effect on INS and INSC-P; while in the control group, the parameters described above increased remarkably. The differences between the two groups were significant ( P〈0.01). No elevated serum LACT and URIC were observed in all groups. No adverse event was reported. Conclusions Adequate intravenous injection of fructose is safe and has little effect on serum glucose and pancreas islet function.
