摘  要:目的为面肌痉挛微血管减压治疗提供解剖学依据。方法对36侧动脉灌注红乳胶的脑标本,在手术显微镜下解剖观察面神经与周围微血管之间的关系。结果①面神经周围的血管70.3%为小脑前下动脉及其分支。②34支面神经有血管接触,共有62个接触点,其中46.8%分布在面神经的内1/3段,14.5%分布在中1/3段,38.7%分布在外1/3段。结论行神经微血管减压治疗面肌痉挛时应兼顾面神经颅内段全程,不能仅行内1/3段减压。Objective To provide an anatomical basis for microsurgical neurovascular decompression for hemifacial spasm. Method Thirty-six sides of adult human brains with red latex injection through the vertebral arteries were dissected and observed under a surgical microscope for the relationship between facial nerve and peripheral microvessels. Results ①Vessels around the cisternal portion of the facial nerve were mostly the anterior inferior cerebellar artery and its branches (70.3%). ② Of the 36 sides of the adult human brain, 34 facial nerves were contacted with arteries, and 62 attachment points of the facial nerve and arteries were observed, 46.8% of which were seen on the proximal 1/3, 14.5% on the middle 1/3, and 38.7% on the distal 1/3 segment of the facial nerve. Conclusion The attention should be paid to the whole cisternai portion besides the proximal 1/3 segment of the facial nerve in neurovascular decompression for hemifacial spasm.
