摘  要:目的观察M-型彩色多普勒超声测定主动脉瓣返流(AR)的血流传播速度(FPV)估测AR程度的可行性及准确性。方法AR患者56例,男34例,女22例,根据彩色多普勒超声综合指标将患者分为轻度、中度和重度AR3组。以M-型色多普勒超声测定AR的FPV,比较3组之间FPV有无显著差别,分析FPV与AR束宽度(JH)、AR束宽度与左心室流出道宽之比值(JH/LVOT)的相关性;以彩色多普勒超声综合指标为标准,评价FPV估测AR程度的可靠性。结果轻度、中度和重度AR3组FPV测值分别为(38.17±11.30)cm/s,(56.04±12.06)cm/s和(84.28±12.88)cm/组间比较差异非常显著(P值均<0.01)。FPV与JH及JH/LVOT比值显著相关(r值分别为0.586和0.601,P值均<0.01)。FPV≥80cm/s为区分重度与中度、轻度AR的截点,特异性100%,预测精确性96.43%;以FPV≤40cm/s为区分轻度AR与度、重度AR的截点,特异性97.06%,预测精确性85.71%。结论FPV是估测AR程度简捷而实用的方法,并具有高度的特异性和精确性。Objective To evaluate the feasibility and accuracy of color M-mode Doppler flow propagation velocity (FPV) in assessment of the severity of aortic regurgitation (AR). Methods Fifty-six patients (34 male, 22 female) with AR were included. They were divided into mild, moderate and severe AR groups based on color Doppler echocardiographic complex parameters. The FPV measurements of AR were obtained with color M-mode Doppler, and their comparisons were performed between the groups. Pearson correlation analysis was used between the FPV and jet height (JH) of AR, as well as JH/left ventricular outflow tract ratio (JH/LVOT) . The reliability of FPV in assessing the severity of AR was studied, using color Doppler echocardiographic complex parameters. Results The FPV values (x^-±s) were (38.17±11.03) cm/s, (56.04±12.06) cm/s, and (84.28±12.88) cm/s in mild, moderate and severe AR groups, respectively (P〈0.01) . A significant correlation was found between the FPV and JH, as well as JH/ LVOT (r=0.586, P〈0.01; and r=0.601, P〈0.01, respectively) .When PFV ≥80cm/s was used asa cut point for seperating the severe AR from the moderate and mild AR, specificity was 100%, accuracy was 96.4% WhenPFV≤40 cm/s was used as a cut point for seperating the mild AR from the moderate and severe AR ,specificity was 97.06%, accuracy was 85. 71%. Conclusions FPV is a simple, practical and reliable method for the quantification of AR with high specificity and accuracy.
