摘  要:目的流出道不畅是背驮式肝移植术后最常见的、最严重的并发症之一,应用三维超声显示移植后的肝脏与下腔静脉的空间位置,以判断流出道的通畅情况。方法将原位肝移植患者分别行二维超声、彩色多普勒检查,测量肝静脉血流速度。同时行三维超声检查,显示移植肝静脉、第二肝门及肝后下腔静脉的血流及空间位置关系。结果(1)三维超声可以显示移植肝肝静脉与第二肝门、肝上下腔静脉、肝后下腔静脉、肝下下腔静脉的空间关系及走行;(2)彩色多普勒显示流出道血流异常的患者,三维超声可以从立体的空间位置显示梗阻的部位。结论三维超声可为背驮式肝移植术后流出道梗阻的诊断提供一种良好的方法。Objective Outflow obstruction is one of the most common and serious complication after piggy back liver transplantation. Using 3-D ultrasound reconstruction to explore the position of transplanted liver and inferior vena cava. Methods Patients after piggy back liver transplantation were studied by 2-D ultrasound and color Doppler flow imaging, to measure blood flow velocity of hepatic vein, and using 3-D ultrasound imaging to explore the position of transplanted liver' s hepatic vein and inferior vena cava. Results 1. The 3-Dimensional can give clear stereoscopic view of the vascular structures which the hepatic vein, liver segment of the inferior vena cava. 2. when color Doppler find abnormity flow of hepatic vein, 3-Dimensional ultrasonic reconstruction can be used to diagnose the diseases of vascular structure itself, to evaluate the blood perfusion of the transplanted organs for diagnosing the regional infarction, to observe the spatial relationship between the vascular tree and the adjacent structure. Conclusions The 3-Dimensional ultrasonic reconstruction can provide very useful information for diagnosis of outflow obstruction after piggy back liver transplantation.
