彩色多普勒超声在下肢交通静脉功能不全诊断中的作用和意义

摘　　要：目的探讨彩色多普勒超声在下肢交通静脉功能不全诊断中的作用和临床意义。方法回顾性分析45例(50条)因下肢静脉性溃疡而行内镜筋膜下交通静脉结扎(SEPS)术的临床资料。术前利用彩色多普勒超声(CDI)检出扩张或功能不全的交通静脉,将其定位与手术结果相比较,以检验彩色多普勒超声诊断的准确性。结果45例(50条)下肢,CDI共检出交通静脉224条。其中一侧下肢检出最少为3条,最多为6条。SEPS术共结扎交通静脉203条,一侧下肢结扎最少为2条,最多为6条,其中183条与CDI术前定位一致,CDI对功能不全交通静脉的诊断符合率为90.1%。结论彩色多普勒超声能准确地对扩张或功能不全的交通静脉诊断和定位,有助于术中寻找需结扎的交通静脉。Objective To explore the role of color Doppler imaging （CDI） in diagnosis of perforating venous insufficiency of the lower extremities. Methods The clinical data of 45 cases （50 limbs） with venous ulcer undergoing endoscopic subfascial perforator surgery were analyzed retrospectively. The dilated or insufficient perforating veins of all the cases were diagnosed and located by CDI. Results In all the 50 limbs of the 45 cases, CDI found out 224 perforating veins, were found on CDI with 3 veins at minimum and 6 veins at maximum in one limb. Totally 203 perforating veins were ligated in SEPS, with 2 veins at minimum and 6 veins at maximum in one limb. Among these veins, 183 were found at the same location as CDI diagnosed. Compared with the results in surgery, the match rate of CDI for diagnosis of dilated or insufficient perforating veins was 90. 1%. Conclusions CDI may correctly diagnose and locate the dilated or insufficient perforating veins, help to find out these veins during operation.
