小梁切除联合羊膜植入术和干扰素α-2b治疗难治性青光眼

摘　　要：目的观察小梁切除术联合同种异体羊膜植入和术眼球结膜下注射干扰素α-2b治疗顽固性青光眼的疗效。方法对23例（23只眼）顽固性青光眼行小梁切除术联合羊膜移植于巩膜床，分别于术时、术后1d、3d、7d、14d术眼球结膜下注射干扰素α-2b 50万单位。术后随访15～25个月，平均18．5个月。观察术后阻止瘢痕性滤过泡形成的效果、降眼压作用、手术成功率及视力变化。结果23只眼中，形成功能性滤过泡20只眼，无功能滤过泡3只眼，术后6个月眼压从术前的（36．42±5．64）mmHg降低到（15．73±2．99）mmHg（t=11．465，P〈0．01）。术后24个月末，手术的成功率为65．8％（寿命表法）。结论小梁切除术联合羊膜植入与干扰素α-2b注射能提高成功率，且安全、可靠，Objective To observe the therapeutic efficacy of trabucullectomy plus amniotic membrane transplantation and subconjunctival injection of interferon （IFN） α- 2b for refractory glaucoma. Methods Twenty-three eyes of 23 patients received trabuculectomy with amniotic membrane transplantation, and then 0.5 million units of IFN α-2b was subconjunctivally injected at surgery, and on day 1, 3, 7 and 14 after surgery. Cicatricial filtering bled, intraocular pressure （ lOP）, operative success rate and visual acuity were observed during the mean follow - up period of 18.5 months （ 15 to 25 months）. Results The functional and non - functional filtering blebs were seen in 20 and 3 eyes, respectively, The mean IOP decreased from pretreatment [ （36.42± 5.64） mmHg] to post-treatment [ （ 15.73 ± 2.99） mmHg] （t = 11.465, P〈 0.01） 6 months after operation. The anterior chamber reaction was mild. The operative success rate was 65.8% 24 months after operation according to life table method. Conclusions The combination of trabuculectomy, amniotic membrane transplantation and subconjunctival injection of IFN α - 2b can not only improve the operative success, but also is safe and applicable for treating refractory glaucoma.
