经颅彩色双功超声检测高血压患者的脑血管反应性

摘　　要：目的:了解不同程度高血压对脑血管血流储备功能的损害。方法:用经颅彩色双功超声(TCCD)检测了33例I级原发高血压(EH)患者(A组),30例II级或III级EH患者(B组),以及38例正常对照组(C组)的安静状态下及屏气25s后大脑中动脉(MCA)的平均血流速度(Vmmca)及其彩色血流宽度(Dmca)。用如下公式计算MCA的血流量(BFVmca):BFVmca(mlmin)=(Dmca2)2·π·Vmmca·60。并把屏气25s后BFVmca的相对增加率作为评价脑血管CO2反应性的观察指标。结果:BFVmca的相对增加率在A组、B组及C组分别为29.07%±3.79%,28.12%±3.97%和31.19%±3.21%。I级EH组的脑血管CO2反应性较正常对照组显著减低(P<0.05),II级或III级EH组较正常对照组减低更为明显(P<0.01)。结论:轻度高血压患者脑血管血流储备能力已明显受损,且随高血压程度的增高损害进一步加重。Objective： To investigate the effects of different levels of hypertension on eerebrovaseular reserve capacity damage. Methods： Thirty-three cases of mild essential hypertension（EH） patients（group A）, 30 cases of moderate or severe EH patients（group B）, and 38 cases of control（group C） were detected using transcranial color-coded duplex Doppler ultrasonogra- phy（TCCD）. The mean velocity of blood flow（Vmca） and the diameter（Dmca） of middle cerebral artery（MCA） were measured at rest and after 25 seconds of breath holding. The blood flow volume of MCA （BFVmca） at rest and after 25 seconds of breath holding were calculated by the following equation： BFVmca（ml/min）=（Dmca/2）%2·π·Vmmca·60. The increase percentage of BFVmca after breath hold was regarded as index. Results： The increase percentage of BFVmca was 29.07%±3.79% in group A, 28.12%±3.97% in group B and 31.19%±3.21% in group C. The capacity of cerebrovascular CO2 reactivity in patients with mild hypertension decreased significantly compared with the controls（P〈0.05）, and it decreased more markedly in patients with moderate or severe hypertension（P〈0.01）. Conclusion： The cerebrovascular reserve capacity damaged even in mild hypertension, and.the damage aggravated with the increased level of blood pressure.
