摘  要:目的:观察内毒素致大耳白兔急性肺损伤过程中的肺微循环障碍及1,6-二磷酸果糖对其的影响,探讨1,6-二磷酸果糖对内毒素所致的兔急性肺损伤的保护性作用。方法:①实验于2003-05/12在解放军总医院呼吸科实验室完成。选用清洁级雄性成年大耳白兔24只。随机将兔分为对照组(静脉注射生理盐水2mL/kg)、致伤组(将内毒素按500μg/kg溶于2mL生理盐水后于5min内经颈静脉一次注射完毕,随后注射生理盐水1次)、干预组[内毒素使用剂量和方法同致伤组,静注内毒素后缓慢静脉给予1,6-二磷酸果糖(300mg/kg)的生理盐水溶液],共3组,每组8只。②各组均于静脉注射内毒素前,静脉注射内毒素后0.5,2.0,4.0,6.0h5个时间点观察血压、心率及呼吸频率变化,并进行采用自动血细胞分析仪和血气分析仪检测各组兔动脉血氧分压及外周血白细胞数量。静脉注射内毒素后6.0h按解放军总医院东亚放免所提供试剂盒说明测肺组织血栓烷B2、6-酮-前列腺素F1α和内皮素1的含量。另取右侧膈叶肺组织采用光镜(×100)和透射电镜做病理学观察。③符合正态分布和方差齐性等条件者做t检验,否则,做秩和检验。结果:兔24只均进入结果分析。①静脉注射内毒素后0.5h,致伤组兔呼吸频率和心率明显高于对照组(P<0.01),血压、动脉血氧分压、白细胞计数明显低于对照组(P<0.01)。②静脉注射内毒素后4.0和6.0h,干预组动脉血氧分压明显高于对照组(P<0.01),致伤组外周血白细胞数量明显少于对照组(P<0.05)。③实验后6.0h,致伤组动物肺组织出现明显的病理损害,肺微血管严重受损。干预组动物肺组织和肺微血管损伤较轻,与对照组相似。致伤组和干预组肺组织血栓烷B2含量明显高于对照组(P<0.01)。干预组肺组织6-酮-前列腺素F1α含量明显高于对照组和致伤组(P<0.01)。结论:①经静脉进入体内的内毒素可通过增加血栓烷B2和内AIM: To study pulmonary microcirculation disorder of endotoxin-induced acute lung injury in flap-eared white rabbits and effects of fructose-1, 6- diphosphate, and explore protective effects of fructose-1, 6-diphosphate on endotoxin-induced acute lung injury. METHODS: ① The experiment was performed at the laboratory of Department of Respiratory, General Hospital of Chinese PLA between May and December 2003. Twenty-four clean-grade flap-eared white male adult rabbits were randomly assigned into control group (2 ml/kg saline injected into vein), injured group (endotoxin 500 μg/kg was dissolved into 2 mL saline, and 5 minutes later it was injected into vein once, following saline was injected once), intervention group (dosage and usage of endotoxin were the same to that in the injured group; And then 300 mg/kg fructose-1, 6-diphosphate saline was given into vein slowly), totally 3 groups with 8 in each group. ② Before injecting endotoxin into vein, at the time points of 0.5, 2.0, 4.0 and 6.0 hours after injecting endotoxin, changes of blood pressure, heart rate and respiratory frequency were observed in every group. Partial pressure of oxygen of arterial blood and number of peripheral blood leucocyte were detected with auto hemocyte analyzer and blood gas analyzer. After injecting endotoxin for 6.0 hours the pulmonary contents of thromboxane B2, 6-keto-prostaglandin F1α (6-ke-to-PGF1α) and endothelins-1 were measured according to test kit provided by Dongya radio-immunity institute of General Hospital of Chinese PLA. Pathologic changes were examined with light microscope (X100) and transmission electron microscope on pulmonary microvessel at the right side. ③ T-test was performed in the patients who were accorded with norreal distribution and homogeneity of variance, etc. Otherwise, the rank sum test was performed.
