PAX2在紫癜性肾炎患儿肾组织的表达及其临床意义

摘　　要：【目的】探讨PAX2在紫癜性肾炎发生中的作用及临床意义。【方法】采用免疫组化法观察30例紫癜性肾炎肾活检组织PAX2的表达情况。【结果】①所有紫癜性肾炎肾病理切片均存在不同程度PAX2再表达,且其中肾远端小管表达明显高于近端小管(P<0.05),足细胞表达微弱,Ⅲa级和Ⅳa级未见足细胞表达。②表达密集部位的肾小管多呈现明显扩张或结构不完整表现。③PAX2在HSPN不同肾脏病理级别的表达积分,无论在近端小管,还是远端小管,各组间差别均无统计学意义。【结论】①紫癜性肾炎患儿肾组织存在PAX2胎儿期回归表达。②紫癜性肾炎PAX2再表达部位主要在肾小管,足细胞表达微弱,其表达程度可能与其病理损害有关。[Objective]To investigate the PAX2 expression in childhood with Henoch-Schoenlein purpura nephritis （HSPN） and try to find the role of PAX2 in the mechanism of HSPN. [Methods]To investigate the expression of PAX2 in kidney tissue of HSPN by immumohistochemistry. [Results]①PAX2 re-expression was showed in all sections of HSPN, more intense staining in distal tubules than in proximal ones, podocytes stained faintly. The podocytes was negative in Ⅲ a and Ⅳ a,normal control .② The tubules, more powerfully expressing PAX2, presented abnormal structure including dilation and/atrophy. ③The PAX2 expressions Ⅱ b, Ⅲa, Ⅲ b, and Ⅳa groups were of no difference. [Conclusion]①There is PAX2 re-expression in HSPN. ② PAX2 re-expression in HSPN is showed more intense staining in distal tubules than in proximal ones, podocytes stain faintly. That suggests the location of PAX2 re-expression is related with pathologic damages of renal tissue.
