Perclose缝合器在冠状动脉疾病诊疗术后的应用

摘　　要：【目的】通过与手工压迫止血比较,评价Perclose缝合器在冠状动脉(冠脉)造影(CAG)和介入治疗(PCI)术后应用的止血疗效和安全性。【方法】172例连续患者分成Perclose缝合器组(80例,其中CAG 44例,PCI 36例)和手工压迫组(92例,其中CAG 50例,PCI 42例)。比较两组的止血时间、制动时间、血管并发症、止血成功率。【结果】与手工压迫组比较,Perclose缝合器组的止血时间[CAG为(4±1)vs(30±3)min;PCI为(4±1)vs(35±10)min,均P<0.001]和制动时间[CAG为(5.1±0.6)vs(13.3±1.3)h;PCI为(6.5±1.2)vs(18.3±2.2)h,均P<0.001]显著缩短,血管并发症显著减少(0 vs 16.3%,P<0.001)。止血成功率相似(96.25%vs 96.74%,P>0.05)。【结论】冠脉诊疗术后用Perclose缝合器止血安全、有效。[Objective]To evaluate the safety and efficacy of Perclose device during hemostasis after coronary arteriography and interventional procedures, comparing with manual compression. [Methods]One hundred seventy-two consecutive patients who underwent coronary arteriography and intervention were divided into Perclose device group （80 cases, coronary arteriography 47 cases and coronary intervention 33 cases） and manual compression group （92 cases, coronary arteriography 56 cases, coronary intervention36 cases ）. The time to achieve hemostasis, the immobile time and vascular complications and success rate of hemostasis were compared between the two groups. [Results]The time to achieve hemostasis[CAG： （4±1） vs （30±3） min; PCI： （4±1） vs （35±10） min, all P 〈0. 001], the immobile time[CAG： （5.1 ±0.6） vs （ 13.3± 1.3） h; PCI： （6.5 ±1.2） vs （18.3±2.2） h, all P 〈0. 001] and vascular complications（0 vs 16.3%） were significantly reduced in Perclose group comparing with manual group. Success rate of hemostasis was similar （97.25% vs 96.74%, P 〉0.05） . [Conclusion]Hemostasis with Perclose device is safe and efficient after coronary arteriography and interventional procedure.
