矽宁治疗矽肺中肺血流图变化分析

摘　　要：目的观察矽宁对矽肺的疗效。方法采用双盲法分组服药。治疗组每日服矽宁片剂300mg,每周服药6天,3个月为一疗程,两个疗程间隙时间1个月,共治疗4个疗程。继续治疗组第5、6疗程日服药剂量为450mg,共治疗24个月;对照组17例,服用安慰剂。用肺血流图观察疗效。结果治疗组的48位病人在6个疗程的指标值进行随机区组设计的方差分析,随时间推移,Zk值、Ha/Hs、Q-B、Q-B/B-Y、Q-BI均有上升趋势(P<0.05),对用药组和对照组在第1、2、3、4、6疗程中的指标值进行重复测量设计的方差分析并结合趋势图分析,发现经方差分析肺血流图各项指标虽P>0.05,但趋势图中仍可见治疗组的改善优于对照组。结论肺血流图能在一定程度上反映经矽宁治疗的矽肺病人的肺血流动力学变化;肺血流图的Hd/Hs值可初步作为临床观察矽肺疗效的灵敏、简单、无创性的指标。Objective To explore the therapeutic effects of Drug Xinin on silicosis. Methods Using the double blind methods, silicosis patients were divided into therapeutic group, continuing therapeutic group and placebo control group respectively. For the therapeutic group, Xinin was administrated with a dosage of 300 mg per day in 6 days a week and lasted for 4 treatment periods （ one treatment period was consisted of 3 months, and there was a break of 1 month between two treatment periods）. For the continuing therapeutic group, Xinin was administrated with the same dosage as the therapeutic group for the first 4 treatment periods, and a dosage of 450rag per day in 5th and 6th treatment periods. The therapeutic effects were assessed by rheopneumography; then the difference of therapeutic effects among these three groups were evaluated by analysis of variance of randomized block design and profile analysis. Results It was found that all of the index values of rheopneumography, such as Zk, Ha/Hs, Q-B, Q-B/B-Y and Q-BI, had an ascent tendency with time going on （P 〈 0.05）. Though the differences were not significant in statistics （P 〉 0. 05）, the therapeutic effects of therapeutic group were better than those of placebo control group. Conclusion The rheopneumography can be used to reflect the changes of lung blood flow dynamics of silicosis patients. The Hd/Hs index can be used as a delicate, simple and woundless index in initial clinical observation on therapeutic effects of silicosis.
