米利酮对大鼠缺氧性肺动脉增压反应的抑制作用

摘　　要：本文观察了磷酸二酯酶抑制剂米利酮（ｍｉｌｒｉｎｏｎｅ，Ｍｉｌ）对大鼠缺氧性肺动脉增压反应的抑制作用。离体肺灌流实验显示，大鼠肺通入２％Ｏ＿２的低氧混合气体后，肺灌流压由基础值的１．８５ｋＰａ（１３．９ｍｍＨｇ）上升至３．６３ｋＰａ（２７．３ｍｍＨｇ，Ｐ＜０．００１）；随灌流液加入０．２，１，５，１０，２０μｇＭｉｌ后，增压反应抑制百分数（下同）分别为３．０±３．３，３４．０±９．５，７０．２±９．４，９７．１±７．４和１１３．７±５．８。整体实验显示，呼吸１０％Ｏ＿２低氧混合气体后，大鼠平均肺动脉压由基础值的２．６１ｋＰａ（１９．６ｍｍＨｇ）升至３．６７ｋＰａ（２７．６ｍｍＨｇ，ｐ＜０．００１）；静脉注入１，５，１０，２０，３０μｇＭｉｌ后，增压反应抑制百分数分别为１２．８±３．１，２４．０±３．３，４６．３±９．７，７０．１±１３．０和９２．９±１３．６。整体大鼠呼吸低氧混合气体后颈动脉血压显著下降，收缩压由１７．６±１．３ｋＰａ（１３２．０±９．６ｍｍＨｇ）降至１０．５±０．６ｋＰａ（７８．６±４．１ｍｍＨｇ，Ｐ＜０．０１）；舒张压由１４．２±１．３ｋＰａ（１０６．９±９．９ｍｍＨｇ）降至６．８２±０．?Inhibitory effect of milrinone on rat hypoxic pulmonary pressor response wasobserved in iso lated lungs and in intact rats res pectively.It was fo und that the perfusionpressure in the rats(n=9) was increased from 1.85 ± 0.08kPa(13.9 ± 0.6mmHg,)to3.63 ± 0.13kPa(27.3 ± 1.0 mmHg,P<0.001)when the inspired gas mixtures were changedfrom normal level(5% CO2 in air)to hypoxia(2% O2 5%CO2 and 93% N2).Afteradministration of 0.2,1,5,10,20μg milrinone,the pereentages of inhibition of hypoxicpulmonary pressor response were 3.0 ± 3.3,34.0 ± 9.5, 70.2 ± 9.4.97.1±7.4 and 113.7±5.8 respectively. The mean pulmonary artery pressure of the rats was raised from 2.6 1±0.12 kPa(19.6 ±0.9mmHg)to 3.67±0.16kPa( 27.6±1.2mmHg,P<0.001) when the intactrats(n=7)were exposed to the hypoxic gas mixtures(10%O2 in N2).After infusion of 1,5,10,20,30μg milrinone through the left femoral vein,the percentages of inhibition of thepulmonary artery pressor response were 12.8 ± 3.2,24.0±3.3,46.3 ±9.7,70.1±13.0 and92.7 ± 1 3.6 respectively.The carotid artery pressure was decreased after hypoxia,andmilrinone showed no significant effect on it.Results showed that milrinone might have aselective pulmonary vasodilatory effect in hypoxic pulmonary hypertension rat and mightbe used for the treatment of pulmonary hypertension.
