经阴道彩色多普勒在药物流产过程中的临床价值

摘　　要：目的通过经阴道超声测量孕囊判断孕龄。观察不同孕龄流产效果,探讨经阴道彩色多普勒对药物流产后宫内残留物的诊断价值。方法通过经阴道超声测量孕囊计算孕龄,将药物流产者分为早早孕组[孕龄30～(d)]及早孕组[孕龄41-49(d)],药物流产 2周后行经阴道彩色多普勒检查,观察宫腔内情况,监测官腔内残留物及子宫肌壁血流情况。结果早早孕组完全流产率明显高于早孕组,不完全流产者官腔残留物内显示血流信号及近宫腔处局部肌壁呈血流丰富区,并测到类滋养层血流频谱。结论行药物流产选择孕龄≤40的早早孕者可提高药物流产成功率。经阴道彩色多普勒对诊断药物流产后的不全流产有重要的诊断价值。Objective To investigate the effect of medical aborting to different gestational age which were estimated by measuring gestational sac through transvaginal ultrasonography and to study the diagnostic value of the intrauterine remainder after drug abortion by transvaginal ultrasound with color Doppler imaging. Methods According to the diameters of gestational sac measured by transvaginal ultrasonography, the pregnant women who volunteered to take medical abortion were divided into two groups, one of which was earlier pregnant group whose gestational ages ranged from 30days to 40days and the other one was early pregnant group whose gestational ages ranged from 41 days to 49 days. All the cases after two weeks were examined with transvaginal ultrasound with color Doppler to Observe uterus and its surrounding myometrial blood flow. Results The complete abortion rate of earlier pregnant group was obviously higher than that in early pregnant group. The intrauterine remainder of incomplete abortion cases showed blood flow signals and trophoblastic blood flow was monitored. Conclusion For medical abortion, the pregnancy of gestational ages should be less titan 40 days, thus the efficacy can be increased.. Transvaginal ultrasound with color Doppler imaging has a considerable diagnostic value in incomplete medical abortion.
