癫癎发作时心率和心电图变化及其影响因素

摘　　要：目的:探讨癫患者癫发作时心率和心电图(ECG)变化及其影响因素。方法:选择在我院癫中心进行录像脑电/心电(VEEG/ECG)监测时有癫发作的患者46例,对其癫发作时的EEG、ECG及其行为学进行分析。结果:在共106次发作中,97次(91.5%)癫发作时心率加快,发作时心率平均增加58.6次/min,癫发作时心率最快达182次/min。19例(41.3%)患者在38次(35.8%)的癫发作时伴有ECG异常改变,主要表现为房性早搏、房室传导阻滞、房颤、心脏停搏、ST段降低、ST段抬高、T波倒置。全身性发作、颞叶癫以及在睡眠中发作三个因素对癫发作时的心率变化有一定的影响。结论:癫患者发作时心率和ECG有明显变化,这种变化可能与癫患者突然意外死亡有关。Objective.. To observe the changes of heart rate and ECG during epileptic seizures in patients with epilepsy and analyze its influencing factors. Methods： One hundred and six-times spontaneous seizures in 46 patients with epilepsy were recorded and analyzed by simultaneous monitoring of the videoelectroencephalogram（VEEG）with one ECG channel. Results.. Ninety-one point five percent （97/106） seizures were associated with an increase of heart rate. The maximal heart rate reached 182 heats/rain. ECG abnormalities occurred by 35.8 % （38/106） of seizures, including atrial premature depolarzations, bundlebranch block, atrial fibrillation, ST-depression,ST-elevation and T-wave inversion. Several factors such as generalized seizures, seizures originating from the temporal lobe and seizures during sleep have signifi- cant effects on heart rate. Conclusions：There were noticeable changes of heart rate and ECG during seizures,which may be related to sudden unexpected death in patients with epilepsy.
