青岛市城区妊娠期糖尿病围产结局分析

摘　　要：目的:探讨青岛市城区妊娠期糖尿病对母婴结局的影响。方法:对来我院检查分娩的3 624例孕妇进行糖筛查,将筛查出的妊娠期糖调节受损(G IGR)184例孕妇中经治疗血糖控制良好者141例作为G IGR1组;血糖控制不好的43例作为G IGR2组;将糖筛查正常的其余3 440例孕妇作为对照组。对孕妇及围产儿结局进行对比研究。结果:G IGR1组妊高征、产后出血、剖宫产率以及新生儿低血糖发病率等明显高于对照组(P<0.01)。G IGR2组妊高征、羊水过多、胎膜早破、产后出血、剖宫产率等明显高于对照组(P<0.01),G IGR2组新生儿低血糖、高胆红素血症、巨大儿等发病率明显高于对照组和G IGR1组。结论:G IGR可对母婴造成多种危害,孕期应积极控制血糖并完善围产期保健,减少母婴并发症。Objective： To study the effects of gestational diabetes mellitus on pregnancy outcome in urban area of Qingdao. Methods： Glucose tolerance test was done in 3624 pregnant women and oral glucose tolerance test performed in those with positive result. 184 pregnant women who suffered from GIGR were screened out.. 141 patients of them were classified group GIGR1 because of their glycemia were well controlled, and the 43 others who suffered from glycemia witch were out of control were classified group GIGR2.. The rest 3 440 pregnant women were to be contrast group. The pregnancy outcomes were compared in group GIGR1, GIGR2 and control group. Results： The incidence of pregnancy induced hypertension syndrome, flooding, cesarean delivery rate and neonatal hypoglycemia in the group GIGR1 was significantly higher than that of the control group （P 〈 0. 01 ） . The incidence of pregnancy induced hypertension syn- drome, polyhydramnios, premature rupture of fetal membranes, flooding, cesarean delivery rate in the group GIGR2 was significantly higher than that of the control group （P 〈 0. 01 ）, as well as the incidence of neonatal hypoglycemia, hyperbilirubinemia, macrosomia in the group GIGR2 was significantly higher than that of the control group and group GIGR1. Conclusion： GIGR can pose adverse effects on pregnant women and neonatal. Perinatal health care is very important for pregnant women. In order to reduce the incidence of syndrome, the pregnant women＇s blood glucose must be well controlled.
