强直性脊柱炎患者HLA-B27、B7、B13、B40抗原表达及免疫学指标的分析

摘　　要：目的探讨强直性脊柱炎患者HLA-B 27、B 7、B 13、B 40的抗原表达,发现抗原相互杂合与交叉反应,以及强直性脊柱炎患者的免疫学指标的变化。方法采用N IH微量淋巴细胞毒法对HLA-B 27等抗原进行检测,抗核抗体采用间接免疫荧光法,免疫球蛋白及补体均采用免疫速率散射法测定。结果126例初诊患者中HLA-B 27阳性率为34.1%,HLA-B 7、B 13、B 40的阳性率分别为6.3%,13.5%,11.1%。HLA-B 27阳性组中HLA-B 13阳性率为27.9%,HLA-B 40阳性率为23.3%,分别高于HLA-B 27阴性组(6.0%,4.8%),而HLA-B 7的阳性率两组无显著性差异。HLA-B 27阳性组的免疫球蛋白、补体、抗核抗体指标均显著高于HLA-B 27阴性组(P均<0.05)。结论检测强直性脊柱炎患者HLA-B 27及相关B抗原有助于疾病诊断和抗原交叉反应的分析,体液免疫异常及自身抗体的出现与HLA-B 27密切相关。Objective To discuss the HLA-B27,B7,B13 and B40 antigen expression of the ankylosing spondylitis and discover antigen＇s mutual hybridization and the cross reaction,along with the change of ankylosing spondylitis＇immunological index. Methods Adopted NIH the method of lymphoid cell poison to detect the HLA-B27,anti-nucleolus antibody was detected with indirect immunofluorescence,immunoglobulin and alexin were detected by the method of rate nephelometry. Results In the 126 patients,the positive rates for HLA-B27,B7,B13,B40 were 34.1%,6.3%,13.5% and 11.1% respectively. In the HLA-B27 positive group the frequency of HLA-B13 was 27. 9% and B40 was 23. 3% ,significantly higher than that in the HLA-B27 negative group （6.0% ,4.8% ,P〈0. 05） ,but in two groups ,no significant difference was found of HLA-B7 positive. In the HLA-B27 positive group,the level and positive of immunoglobulin and anti-nucleolus antibody were significantly higher than that in the HLA-B27 negative group. Conclusion The detection of HLA-B27 and related B antigen in the patients of ankylosing spondylitis is beneficial to diagnosis the disease,and find the antigen cross reaction, the abnormal of humoral immunity and anti-nucleolus antibody are closely related to the HLA-B27.
