摘  要:目的观察甲基强的松龙对大鼠颅脑损伤后脑组织中肿瘤坏死因子(TNF)含量的影响,并探讨其作用机制。方法将75只SD大鼠随机分为3组,采用骨窗形成后硬膜外打击法造成鼠脑挫裂伤模型。手术对照组麻醉后只行开颅手术,不作头颅打击;药物治疗组致伤后立即腹腔内注射30mg/kg甲基强的松龙;治疗对照组则立即腹腔内注射30mg/kg生理盐水。三组大鼠分别在伤后6h、24h、48h、72h、96h时间点断头取脑,对大鼠脑外伤后脑组织中TNF含量和大鼠TNF-α免疫组化反应阳性细胞表达进行检测。结果大鼠脑皮质中的TNF水平在伤后6h后较手术对照组升高显著(P<0.05),48h达高峰,96h降至基础水平。甲基强的松龙药物治疗组在伤后6 ̄48hTNF活性较治疗对照组明显降低(P<0.05)。结论颅脑损伤后,受损脑组织中TNF表达升高。甲基强的松龙可通过抑制损伤后TNF表达,起到保护创伤神经元的作用。Objective To explore the effect of methylprednisolone on the content of TNF in rats' brain tissue after head injury and the related mechanisms. Methods The models of brain contusion and laceration were made by extradural hitting after frontoparictal bone window plasty in 75 rats, and were divided into 3 groups: treatment group (treated with methylprednisolone 30 mg/kg ip after hitting), control group (treated with physiologic saline 30 mg/kg ip after hitting) and normal group (only craniotomy without hitting and injection). The rats were sacrificed and the levels of TNF were measured at different time points (6, 24, 48, 72, 96 h) of time after hitting. Results TNF level in cerebral cortex was higher in the treatment group than that in the normal group at 6th hour after injury, reached to peak at 48th hour, and dropped to the basic level at 72nd hour. The TNF level in methylprednisolone treatment group was obviously lower than that in the control group during 6-48 h after injury. Conclusion The content of TNF increased after head injury. Methylprednisolone is effective in protecting neurons after head injury through suppressed the expression of TNF.
