椎体缺血性压缩骨折的X线、CT诊断

摘　　要：目的:探讨椎体缺血性压缩骨折的X线、CT表现特征及其诊断价值。方法:对30例椎体缺血性压缩骨折的X线、CT表现进行研究,并与100例椎体非缺血性压缩骨折的X线、CT表现进行对照。结果:X线检查30例,显示椎体内横向真空裂隙征24个(78.1%)、空泡征5个(15.6%)、空穴征2个(6.3%)。CT检查10例,显示上述征象者分别为6个(60%)3、个(30%)1、个(10%)。X线正位片真空裂隙宽于侧位片,过伸位或仰卧背部加垫摄片显示更佳。CT除更清楚显示X线所显示的真空裂隙外,尚能显示微小线样裂隙,并真实显示空泡裂隙的数目、病灶范围。100例对照组中无1例显示椎体内真空裂隙征。结论:椎体内空泡裂隙征是椎体缺血性压缩骨折的特征性表现,具有重要的诊断价值。X线与CT结合有助于提高本病的诊断率。Objective：To investigate the X-ray and CT manifestations of ischemic vertebral body collapse and diganosis importance, Methods：X-ray and CT findings of 30 cases with isehemic vertebral body collapse were studied with 100 cases of non-isehemic vertebral body collapse as the control group. Results：Age of the patients ranged from 50 to 89 years with an average of 71 in 30 cases. There were 28 cases （93%） above 60 while there were 26 cases （93%） of women. It usually originate between the eleventh vertebro-stemum and the second vertebro-lumbar （75%）. X-ary demonstrate the transverse vacuum broken sign 24（78.1%） and the bead sign 5（15.6%） and hollow sign 2（6.3% ） of vertebral body. CT demonstrate above sign repectively were 6（ 60% ）, 3（ 30% ）, 1 （ 10% ）. X-ray P-A-position are wider than the lateral in the vacuum broken sign. The vacuum broken sign of 8 cases fail to demonstrate during common posture photograph but it were demonstrate during excessive stretch-posture or be on one＇ s back of put to use back-pad photograph. CT fairly demonstrate the vacuum broken sign bar X-ray demonstrate. Co-demonstrate minute mite-broken of one cases in X-ray fail to demonstrate and truthful demonstrate bead crack number and focal domain. In comparison of 100 cases （traumatic compression-fracture and tubercle and dirty inflammation and tumor） all fail to demonstrate vertebral vacuum broken sign. Conclusion： The vertebral vacuum broken sign is characteristic behavior of ischemic vertebral body collapse and bear important diagnostic merit. X-ray in combination with CT in favor of advance diagnosis-rate of the ischemic vertebral body collapse.
