腰椎管侧隐窝狭窄症的外科治疗（附303例临床分析）

摘　　要：我院从１９８５年１月～１９９４年１０月，外科治疗腰椎管侧隐窝狭窄症３０３例，其中男１９３例，女１１０例，年龄２３～３６岁，平均４４．２岁。根据不同的病理特点，分别采用开窗式、半椎板、全椎板切除等方法，摘除椎间盘髓核，切除肥厚黄韧带，关节突和椎体后缘骨赘，扩大狭窄侧隐窝，神经根得到充分减压。手术证实腰椎管侧隐窝狭窄合并腰椎间盘突出２７５例，单纯侧隐窝狭窄２８例，所有病例经１～１０年随访，平均为５年。结果优２７９例，良１１例，进步１２例，差１例，优良率９６％。From January 1985 to December 1994,303 cases of lateral recess stenosis of lumbar vertebral canal(LRSLVC)were treated by surgical procedure.Among them,there were 193 males and 110 females.The ages ranged from 23 to 76 with an average of 44.2.In the light of the various pathologic condition,enucleation of the discs,excision of the hypertrophic ligamentum flava,excision of the hypertrophic articular process,excision of the vertebral osteophytes,enlargement of the stenosed lateral recesses and decompression of the neural root were accomplished by one of the following approaches:laminectomy, hemilaminectomy or minimal laminectomy.LRSLVC with lumbar disc protrusion(LDP) was found during operation in 275 cases and simple LRSLVC in 028 case.All patients were followed up from 1 to 10 years.The results were excellent in 279 cases,good in 11 cases fair in 11 case and poor in 1 case,The rate of the excellent and good cases was 96%.
