摘  要:用基因工程重组的戊型肝炎病毒(HEV)抗原 ORF3、ORF2和 ORF3-2嵌合抗原及化学合成的 ORF3多肽抗原,分别建立了酶联免疫方法(EIA),检测55份临床可凝的戊型肝炎病人血清中 HEV抗体,以 ORF3-2嵌合抗原检测的抗体阳性率最高,为94.5%(52/55),ORF3重组抗原和多肽抗原检测的抗体阳性率分别为93%(51/55)和91%(50/55),ORF2抗原检测的抗体阳性率为49%(27/55)。上述抗原的检测结果与 Genlabs HEV EIA 检测的结果比较,其符合率相应为100%,98%,96%和52%。32份经 Genlabs EIA 试剂筛选的流行病调查抗体阳性的血清,用四种抗原分别检测 ORF3-2嵌合抗原的抗体阳性数为30例,与 Genlabs 试剂的检测符合率为93.75%;ORF2抗原检出23例,符合率为71.87%;ORF3重组抗原与多肽抗原各检出17例和14例,符合率分别为53.12%和43.75%。正常人群HEV 的抗体阳性率为4%。应用嵌合抗原检测不同症状的肝病患者 HEV 抗体,肝硬化和肝癌者约17%、急性肝炎者15%、慢性肝炎者7%为抗体阳性。Solid-phase enzyme immunoassays(EIA)were developed for detecting anti-HEV immunoglobulin G (IgG)by using recombinant antigens(ORF2.ORF3 and chimeric ORF3-2)and synthetic polypeptide antigen (ORF3)from the open reading frame(ORF)2 and 3 of a Chinese virus strain.The sera were collected from patients with acute viral hepatitis(these patients except two positive for anti-HAV IgM,were negative for anti-HAV,anti- HBV and anti-HCV).Three recombinant antigens ORF3-2,ORF3 and ORF2 were used to detect the specific anti- HEV IgG among 55 HEV suspected acute patients whose HEV-IgG positive rates were 94.5%.93% and 49% respec- tively.91% was positive with the polypeptide ORF3 antigen.2% and 4% were positive in normal human group.The chimeric antigen ORF3-2 was used to detect anti-HEV IgG among clinically different patients with acute viral hepatitis (AH),chronic hepatitis(CAH),cirrhosis and carcinoma of the liver.The positive rates were 14.8%,7% and 16. 7% separately.
