耳击伤后对DPOAEs的影响

摘　　要：目的观察掌击伤鼓膜穿孔后畸变产物耳声发射(DPOAEs)的检出率及幅值的变化情况。方法选择一组门诊所治疗的不合并听骨链中断、耳咽管闭锁及窗膜破裂等症的掌击伤后致鼓膜穿孔患者在受伤后及愈合后分别行DPOAEs等检查并作为外伤组和愈合组。选择听力正常者30例,作为正常对照组,其DPOAEs检测结果与前两组进行统计学分析。结果对照组各频率DPOAEs检出率为83.33%-100%,外伤组为63.33%-100%,愈合组为83.33%-100%。对照组、愈合组各频率DPOAEs幅值分别与外伤组各频率相比均有显著性差异(P<0.05),对照组与愈合组各频率DPOAEs幅值相比均无显著性差异(P>0.05)。结论外伤性鼓膜穿孔后,DPOAEs的检出率在多个频率降低,所有频率幅值下降。穿孔愈合后,DPOAEs的检出率及幅值均恢复正常,说明DPOAEs检测能客观、真实地反映鼓膜穿孔后听力的变化,提示DPOAEs可以作为鼓膜穿孔后听力检测的手段。Objective To investigate the effects of traumatic perforations of the tympanic membrane by spat on DPOAEs. Methods A group of patients with traumatic perforations of the tympanic membrane, no traumatic perilymph fistula and ossicular disarticulation and disfunction of Eustachian tube et al. Were examined by DPOAEs after traumatism and cured. The 30 cases with normal ear functions were selected as nomal group and the DPOAEs were examined. The results were noted and analyzed. Results The rate of appearances of DPOAEs was 83.33 % - 100 % in normal group, 63.33 % - 100 % in the traumatic group and 83.33 % - 100 % in the cured group. The amplitude of DFIDAEs of normal group was almost same with cured group, the amplitude of DPOAEs of traumatic group was lewer than normal and cured groups and there was significant difference. Conclusion Because of the traumatic perforations of the tympanic membrane, the rate of appearances of DPOAEs are fallen at many frequency, the amplitude of the DPOAEs are going down at all frequency. The rate of appearances and the amplitude of the DPOAEs are comeback after the preforations being cured.
