肌钙蛋白I评估心力衰竭严重程度的临床研究

摘　　要：目的探讨心肌肌钙蛋白I(cTnI)对心力衰竭严重程度及预后评估的价值。方法92例充血性心力衰竭病人入院后检测cTnI以及彩色多普勒超声心动图检测左心功能,按cTnI是否阳性分为阳性组和阴性组,比较两组间的心功能分级、病死率、左室射血分数(LVEF)、左室舒张末期内径(LVDd)、心排血量(CO)值的差异。结果阳性组的心功能Ⅲ级、Ⅳ级、病死率均高于cTnI阴性组(P<0.05或P<0.01);彩色多普勒超声心动图测量各项指标阳性组与阴性组比较有统计学意义(P<0.01)。结论cTnI水平可反映心力衰竭的严重程度,可作为评估心力衰竭病人预后的指标。Objective To investigate the value of cardiac troponin Ⅰ on estimating serious degree of congestive heart failure（CHF）. Methods The levels of plasma cardiac troponin Ⅰ were measured in 92 patients （pts） with CHF. The cardiac function was measured by color Doppler echocardiography. Pts were divided to cTnⅠ positive group and cTnI negative group according to the results of cTnⅠ. The cardiac function grade, mortality, left ventricular ejection fraction （LVEF）, end- diastolic left ventricular internal diameter（LVDd） and cardiac output （CO） were compared for two groups. Results The class of cardiac function （ Ⅲ, Ⅳ ） and mortality in cTnI positive group were higher than that in cTnI negative group（P〈0.05 or P〈0.01）. The difference of the heart functions in the cTnⅠ positive group and cTnⅠ negative group tested by color Doppler echocardiography is significant（P 〈 0.01 ）. Conclusion The levels of cTn Ⅰ could reflect severity of heart failure and serve as one of indexes to assess the prognosis of pts with heart failure.
