173例高龄冠心病患者直接经皮冠状动脉介入治疗的临床疗效观察

摘　　要：目的评价高龄冠心病(CAD)患者直接经皮冠状动脉介入治疗(PCI)的安全性、临床疗效。方法回顾分析我院CAD年龄≥70岁173例(高龄组)和年龄<70岁134例(非高龄组)行PCI患者的临床病变特点、手术成功率、手术并发症、近中期临床疗效。结果高龄组行PCI的总成功率与非高龄组比较差异无显著性(99%vs 98%,P>0.05);高龄组死亡率(0.00%)与非高龄组死亡率(1.49%)比较差异无显著性(P>0.05),两组间冠状动脉、外周血管并发症无明显差异;高龄组冠状动脉病变较非高龄组重,支架置入率高于非高龄组(92.6%vs 90.1%)。随访24月,两组间恶性心脏事件发生率无明显差异。结论高龄CAD患者行PCI是安全的,其手术成功率、围手术期死亡率、并发症及近中期疗效与非高龄患者比较无差异。PCI同样是高龄冠心病患者治疗的一种理想选择。Objective To evaluate the safety and clinical efficiency of pereutaneous coronary intervention in elderly patients with coronary artery disease. Methods 173 patients aged≥70 years（ elderly group） who underwent pereutaneons coronary intervention were compared with 134 patients aged〈 70 years（ non-elderly group） in lesion characteristics of coronary artery, successful rates, severe complications and the follow-up results. Results Comparing the technical success rates of pereutaneous coronary intervention and the in-hospital death rate（99% and 0.00% in the elderly group vs 98% and 1.49 % in the non-elderly group）, it showed no significant difference（ P 〉0.05） and complication occurrences in coronary artery and peripheral arteries between two groups did not also. Because the lesion of coronary artery of the elderly group were more severe than that of the non-elderly, stenting rate of the elderly group was higher than that of the non-elderly group. The occurrences of severe heart attack had not significant difference between two group during 6-24 months of the follow-up. Conclusion Pereutaneous coronary intervention would be a safe effective manager for elderly patients.
