长期机械通气并发非结核分支杆菌肺病的诊治

摘　　要：目的分析长期机械通气患者并发非结核分支杆菌肺病诊治过程。方法对1例老年长期机械通气患者并发非结核分支杆菌肺病X线、痰菌、实验室检查及治疗结果进行系统的分析。结果通过X线、痰菌、实验室检查确诊为非结核分支杆菌肺病,给予帕司烟肼(异烟肼+对氨基水杨酸,力克肺疾)、利福喷汀、左氧氟沙星及克拉霉素治疗,X线所示肺部阴影吸收,痰菌转阴。结论长期机械通气除注意呼吸机相关肺炎外,还应注意非结核分支杆菌肺病,给予联合治疗。OBJECTIVE To analyze diagnosis and treatment of nontuberculous mycobacteria in long-term mechanical ventilation. METHODS We systematically analyzed X-rays, bacteria in sputum, laboratory tests and treatment in an elder patient with nontuberculous mycobacteria in long-term mechanical ventilation. RESULTS We diagnosed nontuberculous mycobacteria through X-rays, bacteria of sputum and laboratory tests. The patient was received pasiniazid （INH q- PAS, Dipasic）, rifapentine, levofloxacin and clarithromycin. The X-rays shadow of the lungs and bacteria of sputum were disappeared. CONCLUSIONS The nontuberculous mycobacteria should be paid attention besides ventilator-associated pneumonia in long-term mechanical ventilation.
