摘  要:目的使实验室药敏结果更符合临床实际,指导临床合理用药。方法采用法国生物梅里埃公司ATB细菌鉴定仪及药敏专家系统对超广谱β-内酰胺酶(ESBLs)进行检测,并用NCCLS推荐的确证试验进行比较。结果1 692株革兰阴性杆菌有162株产ESBLs,其中大肠埃希菌和肺炎克雷伯菌检出率最高,ESBLs阳性菌对大多抗菌药物耐药率明显高于ESBLs阴性菌;对三代头孢、氨曲南等抗菌药物表观耐药率并不高。结论应用药敏专家系统能及时有效的检测ESBLs,并对ESBLs阳性菌的药敏结果及时修正,有效控制ESBLs阳性菌株在医院内的播散和流行。OBJECTIVE To get the susceptibility to antibiotics in lab near the clinical truth for reasonable use of antibiotics. METHODS To detect extended-spectrum β-1actamases(ESBLs) by ATB microbiological system and its expert system, and compare with confirmatory test according to NCCLS. RESULTS Totally 162 clinical isolates produced ESBLs from 1692 Gram-negative bacilli strains, detected rate of Escherichia coli and Klebsiella pneumoniae was the hightest. The resistance of ESBLs+ bacteria was higher than ESBLs- ones to most drugs,and the resistance of the ESBLs+ bacteria to the 3rd generation of cephalosporins and aztreonam in vitro was low. CONCLUSIONS Expert system applied to detect ESBLs is satisfactory, it revises the susceptibility to antibiotics quickly, and controls propagated epidemic of ESBLs+ bacteria in the hospital effectively.
