摘 要:[目的]了解肠球菌的感染及耐药情况,同时观察肠球菌感染与抗生素使用的关系。[方法]对大连医科大学第二临床学院2001~2003年的肠球菌感染的临床分布情况进行统计,用“WHONET-4”软件对2003年肠球菌的药敏数据进行分析处理并通过全院电脑联网了解抗生素的使用情况。[结果]2001年共分离肠球菌60株,肠球菌在各类感染标本中的分离率以痰标本最高,占50%。2002、2003年分别分离出肠球菌67、88株,分离率以尿标本最高,分别占31.3%、35.2%。统计2003年肠球菌的药敏结果显示肠球菌对10种常见抗生素除万古霉素和替考拉宁外都有较高的耐药率。对万古霉素和替考拉宁的耐药率分别为13.6%、12.9%。2003年肠球菌感染的88例病例中,应用头孢菌素类抗生素者为70例,占79%。[结论]肠球菌的感染呈逐年上升的趋势,感染部位以泌尿系统为主。目前肠球菌已具有广泛的耐药性,仅对万古霉素、替考拉宁相对较敏感。肠球菌感染的上升可能与临床上大量使用头孢菌素类抗生素有关。[ Objective] To acquire the information about the enterococcal infection and their resistance in hospital,and to observe the relation between the enterococcal infection and the application of antibiotics. [ Methods] Clinical distribution of enterococcal infection in our hospital from 2001 to 2003 were counted. The date from drug sensitivity test of enterococcus isolated in 2003 were analysed using WHONET software and it was by networking in hospital to observe the application of antibiotics. [ Results] A total of 60 strains enterococcus were isolated in 2001, and the isolation rate of enterococcus from sputum was the hightest (50%). 67,88 strains enterococcus were isolated respectively in 2002,2003, and the isolation rate of enterococcus from urine was the hightest (31.3%,35.2% respectively). The results of drug sentivity test of enterococcus show that the resistant rates of enterococcus to 10 commonly used antibiotics except for vancomycin and teicoplanin were all hight. The resistant rates to vancomycin and teicoplanin were 13.6% and 12.9%. Among the 88 cases of enterococcal infection in 2003,70 cases had been treated with cephalosporins (79%). [Conclustions] Infection caused by enterococcus is rising year by year,and the location of infection are mainly urinary system. At prensent,enterococcus has show multdrug resistance to antibiotics and are merely relatively sensitive to vancomycin and teicoplanin. The rise of enterococcal infection may be related with largely using of cephalosporins.
