摘 要:目的研究超重或肥胖型糖尿病及高危对象血清游离脂肪酸谱水平及其与血糖参数间关系。方法采用气相色谱/质谱(GC/MS)联用法测定104例超重或肥胖患者与29例正常对照组血清游离脂肪酸谱水平。结果伴超重或肥胖型糖尿病组血清不饱和脂肪酸(C18:2、C18:1、C20:4、C22:6、C20:3)高于正常对照组,在C18:2、C20:3差异有统计学意义(P〈0.05~0.01);不饱和脂肪酸/总脂肪酸(UFA/TFA)、多不饱和脂肪酸/总脂肪酸(PUFA/TFA)、多不饱和脂肪酸/饱和脂肪酸(PUFA/SFA)显著高于正常对照组(P〈0.01~0.05);饱和脂肪酸(C16:0)和总脂肪酸(SFA/TFA)显著低于正常对照组(P〈0.05~0.01);C18:2、PUFA/SFA显著高于超重或肥胖型糖尿病高危对象组(P〈0.05);C18:2、C20:3、UFA/TFA/PUFA/TFA、PUFA/SFA显著高于单纯超重或肥胖组(P〈0.05~0.01);饱和脂肪酸(C16:0)、SFA/TFA低于单纯超重或肥胖组(P〈0.05)。在研究对象中,血糖与UFA,/TFA、PUFA/TFA、PUFA/SFA呈正相关(r=0.269,0.255,0.307,P〈0.01),与SFA/TFA呈负相关(r=0.269,P〈0.01)。结论超重或肥胖型糖尿病及高危对象中存在游离脂肪酸代谢紊乱。n6系多不饱和脂肪酸升高,可能在其发病中起重要作用。调整膳食结构,可以减少脂质过氧化损伤。Objective To explore the relationship between serum free fatty acid(FFA) spectrum and lasting blood glucose (FBG) in overweight or obese patients with type .2 diabetes mellitus (DM) and risk factors associated with diabetes(RFD). Method The serum FFA spectrum was measured with the gas chromatography and mass spectrometer (GC/MS) in 104 patients and 29 control subjects. Results The DM had higher parameters of unsaturated fatty acid (UFA, C18 : 2=C18 : 1 ,C20 : 4,C22 : 6,C20 : 3), and polyunsaturated fatty acid (PUFA, C18 : 2,C20 : 3) ,UFA/ TFA,PUFA/TFA and PUFA/SFA showed a significant increase as compared with those in the normal subjects (P〈 0. 01- 0.05). Saturated fatty acid (SFA, C16 : 0) and SFA/TFA in DM were decreased remarkably than that in the beahhy controls (P〈0.05-0.01), their C18 : 2 and PUFA/SFA were significantly increased compared to the RFD(P〈 0.05). Subjects with DM had higher ratio of C18 : 2,C20 : 3,UFA/TFA,PUFA/TFA,PUFA/SFA a11d lower ratio of C16 : 0,SFA/TFA compared to the simple overweight or obese patients (P〈0. 05-0. 01). FBG had remarkably positive correlation with UFA/TFA,PUFA/TFA and PUFA/SFA (r=0. 269, 0. 255,0. 307 ,P(0.01, respectively) and a negative correlation with SFA/TFA (r=-0. 269, P〈0. 01) in all of these subjects. Conclusion This study suggests that there was FFA metabolic abnormality in these patients. Increased levels of n6PUFA may play a role in diabetic pathogen esis. Balanced dietary may improve upon the increased lipid peroxidation.
