甲状腺功能减退的彩色多普勒超声诊断

摘　　要：应用彩色多普勒超声对１６例甲状腺功能减退（简称甲减）患者进行了检查，结果表明：甲减患者甲状腺体积明显缩小，与正常人甲状腺体积相比，差异有显著性意义（Ｐ＜０．０１）。甲状腺组织回声减低，彩色多普勒显示血流信号减少，在频谱检测中发现多为静脉频谱，而动脉频谱较难以检出。本组所能检测的甲状腺上动脉的收缩期峰值血流速度（ＰＳ１７．３３±３．４６ｃｍ／ｓ），较正常人组为低（ＰＳ２２．５４±２．５６ｃｍ／ｓ），两者相比差异有显著性意义（Ｐ＜０．０１），而舒张末期血流速度（ＥＤ）和阻力指数（ＲＩ）两组均无显著性差异（Ｐ＞０．０５）。故凡超声发现甲状腺体积明显缩小，回声减低，血流信号减少的患者，结合临床症状和甲状腺激素水平的改变，即可对甲减作出诊断。SixteenthyroidsofpatientswithhypothyroidismweredemonstratedonColorDopplerFlowImaging.Thefindingsindicatedthatthesizeofthyroidofthepatientswithhypothyroidismde-creasedobviously.Itisstatisticalysignificantforthesizetocomparethesepatientswiththenormalgroup(P<0.01).FlowsignalslowedsonColorDopplerFlowImagingand,theechoofthethroiddid.Mostspectrumsdetectedwereofveinsand,venousspectrumsweremoreeasilydetectedthanarterialspectrums.Thereisasignificantdiferenceofthepeaksystolicvelocityofthesuperiorarterialofthethyroidbetweenthepatientsandthecontrastgroup.(PS17.33±3.46cm/svs22.54±2.56cm/s,P<0.01).However,thereisnosignificantdiferenceontheindexoftheendeddiastolicvelocityandtheresistanceindex(P>0.05).Thus,combinedthemanifestationwiththelevelofthyroidhormon,thediagnosisofhypothyroidismcanbesuggestedonColorDopplerFlowImagingwhenthefolowingisfound:decreasedsizeofthyroid,hypoechoofthethyroid,spareflowsignalsinthethyroid.
