超声心动图对左右冠脉分别痉挛时右室功能变化的评价

摘　　要：选用比格犬２３只，随机分左冠痉挛组（ｎ＝７），右冠痉挛组（ｎ＝９）和对照组（ｎ＝７），采用选择性左或右冠脉内注射麦角新碱诱发冠脉痉挛，对照组的右冠代以等量的生理盐水，经超声心动图观察左右冠脉分别痉挛前、后５分、１０分、２０分、３０分、４０分、５０分和６０分钟时右室横径、长径和前壁运动幅度，发现冠脉痉挛后右冠痉挛组的右室横径增加，由痉挛前的１．９２±０．０９ｃｍ增加至痉挛后的２．５８±０．６ｃｍ（Ｐ＜０．０１），大于痉挛后同期对照组（２．２９±０．４ｃｍ～２．５８±０．６ｃｍ／１．９３±０．１１～２．０３±０．１６ｃｍ，Ｐ＜０．０５）；右室长径由痉挛前３．１４±０．０８ｃｍ增至３．６１±０．５ｃｍ，（Ｐ＜０．０５），大于痉挛后同期对照组（３．３５±０．２４～３．５５±０．４５ｃｍ／３．１５±０．０５～３．１６±０．０６ｃｍ，Ｐ＜０．０５）；右室前壁运动幅度较痉挛前降低，由０．８５±０．０１ｃｍ降至０．６９±０．１１ｃｍ（Ｐ＜０．０５），认为左冠或右冠痉挛均可引起右室功能的异常，犬的左冠痉挛对右室功能影响可能更为突出。Inthearticle,23dogsweredividedatrandomintoleftcoronaryspasm(LCAS)group(n=7),rightcoronaryspasm(RCAS)group(n=9)andcontralgroup(n=7).Thediferentcoronaryspasmwasinducedbydog'sleftorrightintracoronryergonovine0.22mg/kg,Controlgroupwasgivenrightintracoronarysaline.Therightventricularhorizontaldiamiter,longdiamiterandanteriorwalscopewereobservedwithechocardiogramatpre-CAS,pre-CAS5min,10min,20min,30min,40min,50min,60min.Aftercornaryspasm,itwasfoundthatrightventricularhorizontaldiamiterincreasedfrom1.92±0.09cmto2.58±0.6cm(P<0.01)inRCASgroupandwasmorethanthatincontralgroup(2.29±0.4～2.58±0.6cm/1.93±0.11～2.03±0.16cm,P<0.05);rightventricularlongdi-amiterincreasedfrom3.14±0.08cmto3.61±0.5cm(P<0.05)andwasmorethanthatincontralgroup(3.35±0.24～3.55±0.45cm/3.15±0.05～3.16±0.06cm,P<0.05);rightventricularanteri-orwalscopedecreasedfrom0.85±0.01cmto0.69±0.11cm,(P<0.01)andlessthanthatcontrolgroup(0.72±0.08～0.69±0.11cm/0.84±0.02～0.82±0.22cm,(P<0.05).BycomparisonwithRCASgroup,rightventricularhorizontaldiamiterandlongdiamiterofLCASgroupwasincreasedmore(2.75±0.26～3.59±0.23cm/2.29±0.4～2.58±0.6cm,P<0.05;3.65±0.25～4.5±0.47cm/3.35±0.24～3.61±0.5cm,P<0.05)andrightventricularanteriorwalscopewasdecreasedmore(0.67±0.08～0.58±0.04cm/0.72±0.08～0.69±0.11cmP<0.05).TheconclutionwasthatLCASorRCASmightinducedrightventriculardysfunctionandthatefectionofLCASonrightventricularfunctionwaslargerascomparedwithRCAS.
