更年期功能失调性子宫出血的B超监测与临床治疗

摘　　要：目的:研究更年期功血与子宫内膜变化的关系。方法:应用B超对206例更年期功血患者的子宫内膜厚度进行监测,根据子宫内膜厚度及阴道出血时间分为A、B、C、D4组。各组再随机分为两组:1组为实验组,按分组进行预定方案的激素替代治疗;2组为对照组,常规给予妇康片治疗。结果:总实验组治愈率高于与总对照组,两组间的治愈率有显著性差异(P<0.005)。其中:A组63例,A1组与A2组比较,P>0.05,两组间的治愈率无显著性差异;B组51例,B1组与B2组比较,P>0.05,两组间的治愈率无显著性差异;C组62例,C1组与C2组比较,C1组的治愈率高于C2组,两组间的治愈率有显著性差异(P<0.01);D组30例,D1组与D2组比较,D1组的治愈率高于D2组,两组间的治愈率有显著性差异(P<0.01)。结论:B超监测更年期功血患者子宫内膜的厚度有益于临床治疗方案的选择。Objective： To study the relation between climacteric dysfunctional uterine bleeding and the change of endometrium , and guide the clinical therapy by the relation. Methods： 206 cases of climacteric dysfunctional uterine bleeding patients which were undertaken monitoring endometrium thickness by B - uhrasonometry. All cases were divided into 4 teams of A , B , C and D according to the endometrium thickness and vagina bleeding time. Every team was randomly divided into two groups again ： groupl was the experimental group which was accepted the hormone therpy （Progynove or Climen） ; group2 was the contral group which was cured by norethindrone. Results： Compared the total experimental and contral group, the cure rate of experimental group was significantly high than that of the contral group （P 〈0. 005） . Among four teams, A team were 63 cases and the cure rate had not significant difference between A1 and A2 group （P 〉 0. 05 ） . And 51 cases of B team had also got the identical result with the A team. In 62 cases of the C team , the cure rate of C1 group was significantly high than that of C2 group （P 〈0. 04 ） ; and in 30 cases of the D group team, the cure rate of D1 group was significantly high than that of D2 group （P 〈 0. 01 ） . Conclusion： It was profitable to select the method of clinical theropy for climacteric dysfunctional uterine bleeding patients by B - uhrasonometry to monitor the endometrium thickness.
