溃疡性结肠炎患者直肠肛门运动及直肠感觉功能

摘　　要：目的研究溃疡性结肠炎(UC)患者肛门直肠运动及直肠感觉改变。方法采用SGY-3多功能消化道检测仪对30例活动期UC、22例缓解期UC患者肛门直肠运动功能及直肠感觉进行测定,以30例健康人为对照。结果活动期和缓解期UC直肠对容量刺激的最低敏感量、最大耐受性、顺应性分别为(68.3±39.4)ml、(170.6±59.8)ml、(23.3±12.6)mlkPa和(74.2±40.3)ml、(190.0±49.3)ml、(28.1±10.1)mlkPa;缓解期UC增加腹压时肛门括约肌净增压、直肠对容量刺激的最低敏感量、最大耐受性、顺应性高于急性期(P<0.01及P<0.05)。结论UC存在肛门直肠运动异常;UC直肠对容量刺激存在高敏感、低耐受、低顺应性和肛门自控能力减弱;缓解期UC直肠肛门运动及直肠感觉功能好转。Objective To study the changes of anorectal motility and rectal sensation in the patients with ulcerative colonitis （UC） during active and no - active period. Methods Anorectal motility and rectal sensation were investigated by multispot balloon probe in 30 patients with active UC,22 patients with no active UC and 30 healthy subjects （KS） were as controlled group. Results （ 1）When abdominal pressure was increased, the not increased pressure of anal sphincter was （ 2.9 ± 1.0） kPa in active UC, （3.1 ± 0.8 ） kPa in no active UC. They were lower than that in KS [ （3.90± 1.58） kPa]（ P 〈 0.05）. （2）The rectal lowest volume of sensory threshold, the maximal volume of tolerance and maximal compliance were （68.3 ± 39.4） ml, （170.6±59.8） ml, （23.3± 12.6） ml/kPa in active UC,（74.2± d0.3） ml,（190.0± 49.3） m1,（28.1 ± 10.1） ml/kPa in no active UC.They were lower than those in KS [ （84.5 ± 45.0） ml, （210.3 ± 46.4） ml, （30.9± 13.2） ml/kPa], P 〈0.01 or P 〈0.05. （3）The rectal lowest volume of sensory threshold,the maximal volume of tolerance and maximal compliance in no active UC were higher than those in active UC （ P 〈 0.01 andP〈0.05）. Conclusions （1）There are some anorectal motility disturbances in UC. （2） Highersensitivity,lower tolerance,lower compliance of rectum and weakened anal automatic control function in UC may be associated with diarrhea and frequent defecation. （3）The anorectal motility and rectal sensation are improved in NAUC.
