诺和灵N联合口服降糖药治疗老年糖尿病的临床观察

摘　　要：目的探讨用重组人胰岛素诺和灵N联合口服降糖药治疗老年糖尿病的疗效与安全性。方法31例符合WHO诊断标准口服降糖药治疗血糖控制不满意的老年糖尿病患者,联合睡前注射诺和灵N。采用自身前后对照,观察31例老年糖尿病患者诺和灵N治疗前后血糖及糖化血红蛋白变化,不良事件及低血糖事件发生情况。结果治疗后第1、2及4周与治疗前比较,空腹血糖及早餐后2h血糖明显下降,统计学上差异有显著性(P=0.000,0.000,0.000),治疗后第4周糖化血红蛋白与治疗前比较,有明显下降,统计学上差异有显著性(P=0.004);在整个试验过程中只有1例75岁女性患者出现轻微低血糖症。结论用重组人胰岛素诺和灵N联合口服降糖药治疗老年糖尿病是安全有效的。[Objective] To discuss the safety and efficiency of recombinant human insulin injection Novolin N and oral hypoglycemia agent combination therapy on elderly diabetes mellitus. [Methods] Included were 31 cases of elderly diabetes mellitus diagnosed by criteria of WHO. All patients had lower effect on oral hypoglycemia agent, were added to given Novolin N injection at bedtime. In this self-contrast analysis, we observed the changes of plasma glucose and HbAlc levels, the incidence of adverse reaction and hypoglycemia events before and after treatment. [Results] Compared with pre-treatment, at 1 week, 2 week,4 week after treatment, the fasting plasma glucose and 2h postpran plasma glucose drop obviously , having significant statistic difference （P =0.000,0.000,0.000）. At 4 week after treatment, HbAlc levels lowered very much, showing significantly statistic difference （P =0.004） （Table 1）; in entire trial only one patient of female 75 years old had slight hypoglycemia. [Conclusion] Recombinant human insulin injection Novolin N and oral hypoglycemia agent combination therapy on elderly diabetes mellitus is safety and effective.
