透析患者脂蛋白(α)水平变化及临床意义

摘　　要：目的:研究血液透析(HD)与腹膜透析(PD)患者血清脂蛋白(α)[Lp(α)]水平的变化及其与心脑血管事件的关系。方法:75例HD患者及50例PD患者的临床及实验室资料作研究,比较两种透析方式对血Lp(α)及脂质水平影响的差别,分析血Lp(α)与其他相关因素特别是与心脑血管事件的关系。结果:PD和HD患者血甘油三酯(TG)和载脂蛋白B(Apo B)均呈显著性增高,血高密度脂蛋白(HDL-C)显著低于正常,以上指标在血、腹透组之间无统计学差异。HD与PD患者血Lp(α)水平均较对照组呈显著性升高(P<0.05),其中PD组较HD组更高(P<0.01);HD与PD患者血Lp(α)水平与超声心动图异常(P<0.05)、ECG异常(P<0.001)及心脑血管事件的发生(P<0.01)、血总胆固醇(TC)及低密度脂蛋白(LDL-C)浓度(P<0.05)呈正相关;而与血浆白蛋白浓度(P<0.05)呈负相关。结论:HD与PD患者脂质代谢紊乱相当常见,以后者更突出,其中以血Lp(α)、TG、HDL-C变化尤为显著,Lp(α)有可能是透析患者并发心脑血管事件的危险因素之一。Objective-To observe the chages of lipoprotein（α） Lp （α） in paients with endstae rena diseae （ESRD） udnergoing hemodiaysis（HD） ad peritonea diaysis（PD）, ad to aayze its relaion with cadiocerebra events. Methods：Clinica daaad laoraory daaof 125 diaysis paients （75 paients on kiD ad 50 paients on PD） were collected ad aayzed. The effects of different diaysis medaities on Lp（α） ad other lipids were compaed ad the roles of Lp（α） in the complicaions were exained. Results：Compaing with the cotrol group, the aerae serum level of TG ad ApoB increaed ad HDL - C decreaed significatly in both PD ad HI） groups. There wa no difference in lipid paaeters between these two groups. The aerae serum level of Lp（α） wa significatly higher in HD ad PD paients tha in the control group（P〈 0.05）, ad the aerae serum level of Lp（α） wa significatly higher in PD group tha tha in HD group;Positive correlaion wa found between the serum level of Lp（α） ad clinica maifestaions（α）norma echoeadiograhy, ECG ad cadia ad cerebra vacula events）. Negaive correlaion wa discovered between the serum level of Lp（α） ad the plamalevel of abumin（P〈 0.05）. Conclusion： Hyperlipidemiais common in diaysis paients, especialy in PD paients. Increaed serum level of Lp（α） ad TG ad decreaed HDL- C ma be risk fators for cadia ad/or cerebra vacula events.
