Univent管在单肺通气中的应用

摘　　要：目的介绍Univent管在胸外科手术单肺通气中的应用经验.方法31例开胸手术中使用Univent管,分别采用盲插法(14例)或纤支镜(FOB)引导法(17例)将支气管阻塞套囊(BB)放置于左或右主支气管中,向BB注入5～6ml空气堵塞主支气管,阻断一侧肺通气.结果31例中除盲插法1例失败外,余30例阻塞套囊均放置成功,所有患者单肺通气后能维持良好的通气和氧合.6例患者术中发生阻塞套囊位置变动,经调整处理后未引起不良后果.结论Univent管能满足胸外科手术要求,具有插入方便、损伤小等独特优点,但在手术过程中应注意观察,及时处理因手术所致BB移位.Objective Introduce the applied experiences of the Univent tube for one lung ventilation （OLV） in thoracic surgical operation. Methods Univent tube was used in 31 cases of thoracic operations, with the blind insertion（14 cases） and the fiberoptic bronchoscope leading （17 cases） respectively. After the bronchial balloon （BB） was put on the left or right main bronchus,it was inflated with 5-6 ml air to block the main bronchus to close the ventilation. Results Except 1 case was failed to place the BB with the blind insertion, the remaining 30 cases were all successful. All cases mainrained the ventilation and oxygenation satisfyingly during OLV. The position of the BB in 6 cases were moved during the operation, but no adverse results occurred after prompt management. Conclusion The Univent tube can satisfy the thoracic surgical operation＇s request, with the special advantages of simple insertion and light mucosal injury etc. But special attention should be paid and prompt management should be done in case any displacement of BB caused by the operation.
