经胸超声结合声学造影在房缺封堵术中的应用

摘　　要：目的探讨经胸超声心动图结合右心声学造影在筛查和指导房间隔缺损封堵术中的应用价值。方法15例拟行经导管封堵术的房间隔缺损患者术前行经胸壁超声结合右心声学造影检查,观察缺损大小、残端及分流情况,术中监测封堵器位置、形态和残余分流。结果11例患者超声观察效果满意,4例行右心声学造影后观察满意。15例患者中2例造影后发现不宜封堵而改行外科手术,余13例一次性封堵成功。1例封堵术后次日出现微量左向右分流,3个月后分流自行消失。结论经胸超声心动图对房间隔缺损的封堵术术前筛查、术中监测和指导有重要的临床应用价值,而右心声学造影对术前筛查有一定帮助。Objective To evaluate the value of transthoracic echocardiography （TTE） combined with right heart contrast echocardiography （RHCE）on occlusion of atrial septal defect（ASD） by Amplatzer septal occluder. Methods The size of ASD and remnant of atrial septal were measured carefully and shunt flow was observed in 15 patients studied before operation by TTE and RHCE. The position and shape of the Amplatzer septal occluder, and the remaining shunt flow were monitored in operation. Results 11 patients were observed satisfactorily only by TTE, while other 4 patients had to be combined with contrast echocardiography. Two of all were given up because of finding of RHCE. 13 cases were treated successfully with Amplatzer devices occlusion. Very small shunt flow appeared in one patient the next day after operation, but vanished voluntarily after three months. Conclusion TIE combined with UC is valuable for selecting patients , monitoring and guiding the procedure. It is very important for clinic.
