摘  要:目的观察慢性再障(CAA)患者血浆雄激素水平和骨髓单个核细胞中雄激素受体的表达,研究雄激素及其受体与CAA病情及治疗反应之间的关系。方法采用放射免疫法测定CAA初发组、CAA治疗组和对照组外周血血浆中的睾酮水平,免疫细胞化学SP法检测三组骨髓单个核细胞(BMMNC)胞浆和胞核内雄激素受体(AR)的阳性表达水平。结果①治疗组外周血血浆睾酮水平显著高于初发组。②CAA初发组BMMNC胞浆和胞核内AR的水平显著低于治疗组和对照组,治疗组AR阳性水平也显著低于对照组。③CAA初发组患者的血浆睾酮含量与其骨髓中的AR水平明显正相关(P<0.05)。④CAA患者骨髓中的AR阳性水平与其外周血中的网织红细胞(RC)百分数正相关(P<0.01),与骨髓非造血细胞百分数负相关(P<0.01)。结论CAA患者发病时BMMNC中AR在胞核和/或胞浆中的表达较正常人减少,因此雄激素刺激骨髓造血的作用减弱。检测CAA发病时AR的表达量可能在CAA的病情监测方面有一定意义。Objective To observe the level of testosterone in plasma and androgen receptor expression in bone marrow mononuclear cells, and the correlation of androgen receptor and androgen with patients, state and therapeutic response. Methods ①Radioimmunoassay was used to investigate the contents of testosterone in plasma of patient with initially diagnosed chronic aplastic anemia (CAA), CAA patients treated by androgen for three months and healthy controls. ②Immunocytochemistry was used to evaluate the level of androgen receptor in bone marrow mononuclear cell plasma and nucleus in the three group patients. Results The level of testosterone in plasma of patients after treating with androgen was higher than that of initially diagnosed ones. The contents of androgen receptor in bone marrow in initially diagnosed group were significantly lower than those of treatment and control groups, and those of treatment group were also lower than control group. There was positive correlation between testosterone and androgen receptor contents in initially diagnosed patients(P 〈 0.05). In addition, there was positive relationship between peripheral blood reticulocyte count and bone marrow androgen receptor. Conclusion The expression of androgen receptor in bone marrow mononuclear cell plasma and nucleus in patients initially diagnosed is less than those in healthy subjects, so the androgen function to stimulate hematopoiesis in bone marrow is reduced. It may be valuable for identifying the expression of androgen receptor when they were initially diagnosed as CAA.
