摘 要:目的探讨葡萄糖筛查假阳性有无临床意义。方法通过对1500例无合并症的孕产妇资料进行回顾性研究,其中葡萄糖筛查假阳性499例。结果葡萄糖筛查假阳性组孕妇平均年龄为29.4±2.6(岁),对照组为28.5±2.8(岁),有差异(P<0.01)。新生儿平均出生体重假阳性组为3443.89±428.30(g),对照组为3390.50±401.86(g),也有差异(P<0.05)。两组剖宫产率分别为37.27%和35.56%,巨大儿发生率为9.82%和8.79%,无统计学差异(P>0.05),产钳助产率为13.43%和14.49%,无差异(P>0.05)。胎儿窘迫发生率分别为34.87%和40.96%,低于对照组(P<0.05)。孕24周时体重指数≥24,葡萄糖筛查假阳性组,巨大儿的发生率为25.35%,明显高于对照组(9.92%),有显著差异(P<0.01)。结论葡萄糖筛查假阳性可影响新生儿出生体重,但不增加剖宫产和产钳助产的危险,不会造成胎儿宫内缺氧。孕24w超重的孕妇,糖筛查假阳性巨大儿的发生率增加。Objective:To determine effects of a false- positive 1 - hour glucose screening test on the pregnant women and fetus. Methods 1500 eligible pregnant women were retrospective studied. The 499 women were defined as a result greater than or equal to 7.8mmol/L of 1 - hour glucose screening test followed by a normal 3 - hour glucose tolerance test. Results:The average age of the false- positive group(29.4 ± 2.6) was older than the negative group( P 〈 0.01), and the birth weight(3443.89 ± 428.30(g)was higher than the negative group( P 〈 0.05). The pregnant women of false - positive group whose body mass index had greater than or equal to 24 in 24 weeks more frequently had macrosomia. Conclusion:There is more birth weight associated with a false - positive 1 - hour glucose screening test. Risk of macrosomia increased with BMI≥24 in 24 gestational weeks.
