摘  要:目的:进一步明确胆汁各主要有机成分在胆囊结石形成过程中的作用.方法:分别用正常饲料、致石饲料饲养对照组和致石组豚鼠.在饲养过程中的不同阶段动态观测豚鼠胆囊胆汁中总胆固醇、粘蛋白、总磷脂和总胆汁酸含量的变化,并在偏光显镜下观察各阶段成石情况.结果:对照组豚鼠胆囊胆汁中的总胆固醇、粘蛋白、总磷脂和总胆汁酸的含量在整个饲养过程中基本保持不变,且胆汁中未见有形结石晶体出现;而致石组豚鼠胆囊胆汁中总胆固醇和粘蛋白含量逐渐升高,总磷脂和总胆汁酸含量却逐渐降低,与对照组相比,饲养10 d后就有显著性差异(P<0.02,P总胆固醇<0.001,P粘蛋白<0.02),饲养25 d后就有非常显著性差异(P<0.001),且随着饲养天数的增加,致石组豚鼠胆汁中结晶物增多变大并最终形成大量微结石晶体.结论:在胆囊结石形成过程中胆固醇起到了成核的作用,粘蛋白起到了促成核的作用,而磷脂和胆汁酸起到的则是抗成核作用.Objective: To further study the role of main organic substances of bile in the process of gallbladder stone formation. Methods: Normal feeding and stone - leading feeding were used respectively to raise guinea pigs in the control group and stonecausing group. The dynamic changes of total cholesterol, mucoprotein, total phospholipid and total cholic acid were measured during the various raising period. The formation of stone was studied under a polarzing microscope. Results: The contents of total cholesterol, mucopro- tein, total phospholipid and total cholic acid in the gallbladder bile of control group were almost the same during the whole feeding process, and there was no shaped stone crystal formation. For the stone - causing group, however, the contents of total cholesterol and mucoprotein gradually went up and the contents of total phospholipid and total cholic acid gradually went down. With the increasing of feeding days, the crystal substance in the bile increased and became bigger and the number ot microcrystais formed further.Conclusion; During the process of gallbladder stone formation, the cholesterol played the role of shaping the nucleation and mucoprotein was a facilitating factor and could help bring about the nucleation; while phospholipid and cholic acid played the role of anti - nucleation.
