小儿梅克尔憩室异位胃黏膜的分布及其临床意义

摘　　要：目的　探讨小儿梅克尔憩室 (MD)异位胃黏膜 (HGM)的分布特点及其临床意义。方法　将 36例手术切除MD(2 1例含HGM ,15例不含HGM)均做HE染色及免疫组化 :抗actin检测。结果　HGM分布于MD顶部 2 1例 ,其中占据MD全段者 1例。HGM处肌层为三层者 9例 ,二层者 7例 ,单层者 5例 ,肌层稀疏者 8例 ;不含HGM的MD肌层为二层 ,内为环肌 ,外为纵肌 ,未见肌层稀疏。抗actin检测示 :HGM处肌层不同程度的局灶性actin消失或呈浅表达 ,不含HGM的MD表达均呈阳性。结论　HGM均分布于MD顶部 ,个别病例可占据MD全段 ;HGM处三层肌层结构的病理改变 ,可能因胃组织异位所致 ,而不仅是胃黏膜异位 ;HGM的这种分布特点对手术有参考意义。Objective To evluate the distribution pattern of heterotopic gastric mucosa （HGM） in children with Meckel＇s diverticulum（MD）. Methods The specimens from 36 patients with MD were pathologically analyzed after staining with hematoxylin and eosine. Moreover, all specimens also underwent to immunohistochemical staining for actin. Results There were 21 cases with HGM while 15 cases without. All of HGMs were noted at the tip of MD, and HGM could be found in all area of MD in one ease. The muscularis distribution was three layers in 9, two layers in 7, monolayer in 5, and sparse in 8. Muscularis in MD without HGM, however, was mainly composed by two layers. The expression of actin was decreased in muscularis at the site of HGM compared to MD without HGM. Conelusions Our data suggested that the pathological changes of the muscularis at the site of HGM may be caused by heterotopic gastric tissue rather than by single HGM.
