绝经后骨质疏松症妇女体脂成份、下肢肌力和血清雌激素、维生素D的关系

摘　　要：目的调查分析绝经后骨质疏松症妇女体脂成份、下肢肌力和血清雌激素、维生素D的关系。方法上海市区50～70岁社区绝经后女性,经DEXA骨密度检测,符合骨质疏松症诊断的共72例,应用MES肌肉骨骼运动分析系统进行体脂成份(含量)、下肢肌力的检测,同时进行应清雌激素(雌二醇)和维生素D(25OHD)的测定。应用SPSS软件进行统计分析。结果下肢肌力与血清雌二醇的相关系数为0.068,而下肢肌力与血清维生素D的相关系数为0.095;体脂成份(含量)与血清雌二醇的相关系数为0.140,而体脂成份(含量)与血清维生素D的相关系数为0.086。但这些相关系数均无统计学意义。下肢肌力较高的A组,其血清雌二醇和清维生素D水平明显高于下肢肌力较低的B组(P<0.05)。体脂成份(含量)较高的Ⅰ组,其血清雌二醇水平明显高于体脂成份(含量)较低的Ⅱ、Ⅲ组(P<0.05)。但血清维生素D水平在这3组中非常接近,没有统计学差异。结论本研究的结果提示雌激素对脂代谢和肌肉功能均有调节作用,而维生素D仅对肌肉功能起作用。对骨质疏松症患者,根据患者血清雌激素和维生素D的浓度来选择治疗药物,同时可参照患者的体成份和肌力等状况进行综合考虑。Objective To investigate the relationship among body fat（BF）, low extremity muscle strength（LEMS） and estrogen （ E2 ）, vitamin D （ VD ） in osteoporotie postmenopausal women. Methods Seventy-two menopausal women aged 50 - 70 from communities in Shanghai were collected and diagnosed osteoporosis by DEXA. Their bone density, BF and LEMS were measured, and the consistency of E2 and VD （25-OH-D）were determined. The date was statistically analyzed with SPSS software. Results The correlation coefficient between LEMS and E2 was 0.068. However, it was 0. 095 between LEMS and VD . The correlation coefficient between BF and E2 was 0.140 and 0.085 between BF and VD . None of the above coefficients had statistical significance .The values of E2 and VD in the group with high LEMS were higher than that with low LEMS（ P 〈 0.05） .While E2 level in the group with high BF were higher than that with low BF（ P 〈 0.05）. There was no difference in VD level between the different BF groups. Conclusions Estrogen can regulate not only the metabolism of the fat, hut also the function of muscle. However, vitamin D can only influence the function of muscle. Osteoporosis treatment selection should be based on the value of BF, LEMS, E2 and VD .
