国产网状尿道支架在前列腺增生症的应用及疗效观察

摘　　要：目的研究国产网状尿道支架治疗良性前列腺增生(BPH)所致排尿困难的安全性及疗效. 资料与方法对23例70～82岁BPH引起的排尿困难的患者,在X线监视下置入25枚国产网状尿道支架治疗. 结果 21例分别置入1枚支架,2例因前列腺尿道部过长,分别置入2枚支架,支架放置后均立即自行排尿.15例术后随访6～24个月,其中2例分别于术后第10、14个月出现尿线变细,1例于术后第20个月出现排尿困难.国际前列腺症状评分(I-PSS)术前24.7±5.1,术后5.3±0.4(P＜0.01),术前最大尿流率(6.2±3.3) ml/s,术后(14.1±0.6) ml/s(P＜0.01). 结论国产网状尿道支架治疗BPH安全有效,特别适用于不能耐受手术的高龄BPH引起的排尿困难患者.Objective To study the curative effect ＇of domestic urethral wallstent on dysuria caused by benign prostatic hypertrophy. Materials and Methods 25 domestic urethral Wallstents were introduced into 23 patients（aged between 70 and 82 years old） who suffered from dysuria caused by benign prostatic hypertrophy, The operations were performed under X-ray monitoring. Results 21 stents were introduced into 21 patients respectively, 2 stents were placed in 2 patients respectively due to overlong urethral part of the prostate, All patients began to urinate themselves instantly after the operation, 15 patients were followed up for a varying period of 6 ～24 months, among which, 2 patients presented with thinning of the urinary stream on the 10th and 14th month after the operations respectively, 1 patients presented with dysuria on the 20th month after the operation. The I-pss score is 24.7±5.1 before operation and 5.3± 0.4 after the operation （ P 〈 0.01 ）, PUFR were （6.2 ± 3.3） ml/s and （ 14.1±0.6） ml/ s （ P 〈 0.01 ） respectively before and after stenting, Conclusion Domestic urethral wallstent is a effective and safe treatment of benign prostatic hypertrophy, especially for patients with advanced age who suffer from dysuria and can not endure surgical operation.
