超声背向散射积分技术评价高血压病患者的左室舒张功能

摘　　要：目的研究超声背向散射积分周期平均变化幅度(ACVIB)与高血压患者左室舒张功能的关系及评估高血压患者左室舒张功能的可行性.方法高血压病117例和对照30例,运用IBS测量室间隔和左室后壁的心肌背向散射积分周期变化幅度(CVIB)并计算左室平均CVIB(averaged CVIB,ACVIB),采用2-DE,PW,PW-DTI及彩色M型测量左室功能.结果根据E/A,IVRT,DT将高血压病患者分为4组舒张功能正常组(组1,n=34)、松弛异常组(组2,n=28)、充盈假性正常化组(组3,n=36)、限制性左室充盈障碍组(组4,n=19).组1与对照组ACVIB差异无显著性,而舒张功能异常组(组2,3,4)与对照组相比,ACVIB逐渐减低.线性相关回归分析表明研究对象ACVIB的变化与Ea,Ea/Aa,Vp呈正相关(r分别0.66,0.72,0.70,P<0.01).结论ACVIB的减低与舒张功能的减退密切相关,观察左室心肌ACVIB的变化,可评估高血压病患者的左室舒张功能.AIM： To study the relationship between averaged cyclic variation in integrated backscatter （ACVIB） and the diastolic function of left ventricle in patients with hypertension and to explore the feasibility of using ACVIB as an indicator of left ventricular diastolic function. METHODS： By means of IBS, left posterior ventricular myocardial CVIB was measured in 117 essential hypertensive patients and 30 normal subjects as control group. The left ventricular func- tion was measured at the same time with 2-DE, PW, PW- DTI and color M-mode echocardiography. RESULTS： Essential hypertension patients were divided into 4 groups according to E/A, IVRT and DT： normal diastolic function （group 1, n = 34）, delayed relaxation pattern （group 2, n =28）, pseudonormal filling pattern （group 3, n=36） and left ventricular restrictive filling pattern （group4, n = 19）. There was no difference in ACVIB between patients with normal diastolic function and the controls. ACVIB decreased gradually in patients with abnormal diastolic function. ACVIB of left ventricle was significantly correlated with Ea, Ea/Aa and Vp （r=0.66, 0.72, 0.70, P〈0.01）. CONCLUSION ： There is an association between the diminished ACVIB and the deterioration of diastolic function in hypertensive patients. Alterations in this parameter may be useful for the assessment of diastolic dysfunction in hypertension.
