摘 要:目的探讨早产儿肠系膜上动脉(SMA)血流变化与喂养耐受性的关系,以便科学地指导早产儿喂养护理,减少并发症的发生。方法将40例单纯早产的早产儿按喂养耐受情况分为观察组(喂养耐受,11例)和对照组(喂养不耐受,29例),两组新生儿采用多普勒超声测量第1次喂养前后SMA血流动力学参数,即收缩期峰值流速(PSFV)、时间平均流速(TMFV)、舒张末期流速(EDFV)、搏动指数(PI)、阻力指数(RI),并记录足量喂养时间。结果观察组喂养前后SMA血流动力学监测结果比较,差异有显著性意义(P<0.01或P<0.05);两组喂养后TMFV、RI结果比较,差异有显著性意义(均P<0.05);两组喂养后TMFV的增加与达足量喂养时间呈显著负相关(r=-0.34,P<0.05);观察组达足量喂养时间显著短于对照组(P<0.01)。结论喂养可改变早产儿SMA血流动力学参数,血流动力学参数的改变与喂养耐受密切相关,医护人员可根据第1次喂养前后SMA血流动力学参数的改变,特别是TMFV的增加来预测喂养风险,从而科学地指导早产儿喂养护理。Objective To study the relationship between superior mesenteric artery(SMA) blood flow velocity changes and tolerance to enteral feeding in premature in order to direct nursing care of prematrue feeding scientifically and decrease the occurrence of complications. Methods According to fecding tolerance, 40 prematures were divided into observation group (n= 11 ) and control group (n=29). Doppler ultrasound was used to measure SMA blood flow hemodynamic parameters including: PSFV (cm/s), TMFV (cm/s), EDFV (cm/s), RI and PI before and after the first feeding in 40 prematures. The time of full enteral feeding was recorded. Results There was significant difference in blood flow velocity before and after feeding in observation group (P〈0.01 or P〈0.05). After the first feeding, there was significant difference in TMFV and RI between the two groups (all P〈0.05), and there was a negative correlation between the increased TMFV and the time of full enteral feeding after first feeding in the two groups (γ= -0.34, P〈 0.05 ). There was a sigaificant difference in the time of full enteral feeding between the two groups (P〈0.01). Conclusion Feeding can change SMA blood flow velocity in premature. The changes of blood flow parameters have a close correlation with feeding tolerance. The clinical staffs can predict risk of feeding through measuring SMA blood flow hemodynamic parameters at the first feeding, especially the increase of TMFV.
