肥厚型心肌病患者左房射血力变化的超声研究

摘　　要：目的:评价肥厚型心肌病(HCM)患者的左房射血力并探讨其意义。方法:使用超声技术测量56例HCM患者和33例正常对照的左房射血力,根据HCM患者左室舒张功能受损程度不同依次分为Ⅰ、Ⅱ、Ⅲ、Ⅳ组。结果:左房射血力,Ⅱ、Ⅲ组高于正常对照组、I组和Ⅳ组,其中Ⅱ组又高于Ⅲ组(P均<0.05);肺静脉口A波峰值流速,Ⅱ、Ⅲ、Ⅳ组明显高于正常对照组及I组,其中Ⅱ组又高于Ⅳ组(P均<0.05);左房泵血量,Ⅱ、Ⅲ组高于正常对照组(P均<0.05)。结论:多数肥厚型心肌病患者左房射血力代偿性增加,左房射血力受心室顺应性等因素影响。Objective： To evaluate the left atrial ejection force （LAEF） in patients with hypertrophic cardiomyopathy （HCM）. Methods： The LAEF in fifty-six patients with HCM and thirty-three normal control subjects were estimated echocardiographically. Patients with HCM were divided into four groups （Ⅰ,Ⅱ,Ⅲ,Ⅳ ） according to the severity of left ventricular diastolic dysfunction. Results： The LAEF force in groups Ⅱ, Ⅲ were significantly higher than that in other groups while it was higher in group Ⅱ than that in group Ⅲ （all P〈0.05）.The peak velocity of atrial reveral flow in pulmonary, vein were higher in groups Ⅱ,Ⅲ,Ⅳ than those in group Ⅰ and normal controls ,while it was higher in group Ⅱ than that in group Ⅳ （all P〈0.05）. The left atrial ejecting volume in groups Ⅱ,Ⅲ were higher than that in controls （all P〈0.05）. Conclusion： The LAEF in most HCM patients, which is influenced by some factors such as left ventricular complicance, has compensatory enhancement.
