腹部钝性损伤致腹膜后血肿

摘　　要：本文回顾性分析了从１９８２年２月～１９９２年１１月间２２８例腹部钝性损伤。手术治疗的１８９例中，发现腹膜后血肿４９例，手术探查３８例，探查率为７７．６％，２８例证实有脏器伤：其中胰腺伤８例，肾脏伤９例，十二指肠伤７例，大血管伤２例，４９例中，Ⅰ区血肿３８例，Ⅱ区７例，Ⅲ区２例，Ⅳ区２例。Ⅰ区３２例作了手术探查，阳性发现２４例。４９例中死亡１０例，病死率为２０．４％。我们认为：腹部钝性损伤所致腹膜后血肿，Ⅰ区血肿需探查，并及时处理受伤脏器，可降低病死率。patients with blunt trauma of abdomen admitted into our hospital from February 1982 to November-1992 were reviewed retrospectively,Among the 189 cases operated,49 were found to have retroperitoneal hematoma(RPH).77.6%of these 49 cases were explored in. Injuried to the retroperitoneal organs were fOund in 28 cases:8cases in pancreas,9 in kidney,7 in duodenum and 2 in major vessel;Among 49 cases with RPH,38 were located inZone Ⅰ,7 in Zone Ⅱ,2 in Zone Ⅲ and 2 in Zone Ⅳ. Among 32 cases with RPH in Zone Ⅰ explored,24 were foundto be positive. The over-all mortality rate was 20.4%。The result shows the most of the RPH in Zone Ⅰ resultingfrom abdominal blunt trauma need exploration.
