摘 要:目的探讨原发性高血压患者高频切迹(highfrequencynotches,HFN)数与心肌损害程度的相关性。方法高血压病患者68例,作超声心动图检查,分为高血压组、高血压左心室肥厚组、高血压左心室肥厚扩大组,对照组为正常人24例。按等级资料对HFN出现0~6,7~11,12~20和20~记录病例数。结果高血压病患者出现HFN数明显多于正常人(!2=42.593,P<0.01,n=92);高血压左心室肥厚及扩大组HFN数明显大于高血压组及高血压左心室肥厚组(!2=23.64,P<0.01,n=68);高血压左心室肥厚扩大组HFN数亦高于高血压左心室肥厚组,(!2=15.594,P<0.01,n=42)。结论高血压患者HFN数较正常人显著增多,与高血压患者左心室损害程度及其心脏病理发展过程呈明显一致性。HFN数对于判断高血压患者左心室损害程度有一定的临床价值。Objectives To explore correlations between the number of high frequency notches (HFN)and the degree of myocardial lesion in patients with primary hypertension (HBP). Methods According to the data of echocardiography, 68 patients with HBP were divided into three groups: HBP group, HBP with left ventricular hypertrephy(HBP+LVH)group,HBP with left ventrieular hypertrophy and dilation (HBP+LVD)group. 25 healthy subjects were served as controls. The number of HFN was recorded in all subjects. Results The number of HFN significantly increased in three hypertensive groups compared with in the control group (P〈0.05 or P〈0.01 ). The HBP+LVD group showed markedly higher HFN number than the HBP+LVH and HBP group(P〈0.01),the latter 2 groups were statistically compared despite that the HBP+LVH group obtained mildly increased HFN number (P 〉 0.05 ). Conclusions (1) For hypertensive patients, the number of HFN gradually increased in agreement with their degree of myocardial lesion and cardiac pathological progress. (2) The number of HFN might be employed as a useful parameter for evaluating the left ventricular lesions in patients with hypertension.
