慢性乙型病毒性肝炎患者血清瘦素水平与肝纤维化的关系

摘　　要：目的观察慢性乙型病毒性肝炎患者血清瘦素水平与肝纤维化的关系及其在肝纤维化中的作用。方法 82例经肝穿病理证实的慢性病毒性乙型肝炎住院患者,测定其体积指数,用 ELISA 法检测血清瘦素水平,用 RIA 法检测肝纤维化血清指标如透明质酸(HA)、Ⅳ型胶原(CIV)、Ⅲ型前胶原(PCIII)、层粘蛋白(LN)。结果纤维化分期为0～4期的患者血清瘦素水平分别为(4.21±2.96)ng/ml,(4.82±4.48)ng/ml,(6.30±5.96)ng/ml,(7.97±4.52)ng/ml,(10.08±5.18)ng/ml,差异显著(F=2.629,p=0.041);女性患者的血清瘦素水平(8.51±5.35)ng/ml 显著高于男性患者的(5.25±4.73)ng/ml(t=2.784,p=0.007);血清瘦素水平与肝纤维化程度呈正相关,其相关系数为0.277(p=0.012)。结论在慢性乙型病毒型肝炎患者,随着肝纤维化的进展,其血清瘦素水平亦相应升高,肝纤维化的程度与血清瘦素水平正相关。Objective To investigate serum leptin levels in relation to severity of liver fibrosis in patients with chronic hepatitis B, and to determine the effect of leptin in the process of liver fibregenesis of chronic hepatitis B. Methods Eighty - two patients with chronic hepatitis Bwere included in this study. Serum leptin levels were determined by means of enzyme - linked immunosorbent assay （ELISA）; Serum markers of liver firosis, such as hyaluronic acid, collagen type Ⅳ, procollagen type Ⅲ, laminin, were determined by radioimmunoassay （RIA）. Remits Serum leptin levels in patients with chronic hepatitis B increased significantly with the severity of liver fibrosis （F = 2.629, p = 0.041）, and they were （4.21±2.96） ng/ml, （4.82±4.48） ng/ml, （6.30±5.96） ng/ml, （7.97±4.52） ng/ml, （10.08±5.18） ng/ml in liverfibrosis stage 0～4 patients respeetively. Serum leptin levels of female patients were （8.51±5.35） ng/ml, significantly higher than those of male patients [（5.25±4.73） ng/ml] （t = 2.784, p = 0.007）. Serum leptin levels were positively related with the severity of liver fibrosis （r = 0.277, p = 0.012）. Conclusion In chronic hepatitis B, sennn leptin levels increased along with progress of liver fibrosis and were positively related with the severity of liver fibrosis, these showed that leptin played a role in the process of liver fibrngenesis of chronic hepatitis B.
