氯沙坦和卡托普利联合治疗糖尿病肾病的临床疗效观察

摘　　要：目的探讨氯沙坦和卡托普利联合治疗糖尿病肾病(DN)的有效性及安全性。方法59例2型糖尿病肾病患者随机分为氯沙坦和卡托普利联合治疗组(n=29)及卡托普利组(n=30),疗程均为1年。观察治疗前后两组收缩压(SBP)、舒张压(DBP)、24h尿蛋白、血尿素氮(BUN)、血肌酐(Cr)、空腹血糖(FBS)、糖化血红蛋白(HbAlc)、血钾(K+)、总胆固醇(CH)、甘油三酯(TG)及药物不良反应。结果联合治疗组和卡托普利组均有明显降压、减少尿蛋白、降血尿素氮、血肌酐的作用,两组治疗前后比较差异有显著性(P<0.01、P<0.05);联合治疗组尿蛋白降低程度与卡托普利组比较,差异有显著性(P<0.05);联合治疗组SBP、DBP、BUN、Cr降低幅度较卡托普利组更为明显;两组FBS、Hbalc、K+、CH、TG治疗前后比较无统计学意义(P>0.05)。结论氯沙坦联合卡托普利治疗糖尿病肾病更有效、更合理,无不良反应。Objective To evaluate the efficacy and safty of losartan combined with captopril in patients with diabetic nephropathy （DN）. Methods Fifty - nine patients with DN were randomized into two groups ： group A received losartan and captopril （n = 29） ; group B were given captopril （ n = 30 ） for one year. Systolic blood pressure （ SBP）, diastolic blood pressure （DBP）, 24 hours urinary protein, serum BUN, Cr, FBS, HbAlc, potassium （K^＋ ）, CH, TG and side effects of the above drugs were observed. Results There were significant differences （P〈 0.05 or P 〈 0.01 ） in decreasing SBP, DBP,urinary protein, BUN and Cr after treatment in both groups. As compared with group B, urinary protein were decreased markedly in group A （P 〈 0.05）. The decreasing degree of SBP,DBP,BUN ,Cr in group A was bigger than those in group B. There was no difference in serum levels of FBS, HblAc, potassium, CH and TG before and after treatment between two groups （P 〉 0.05 ）. Conclusion Losartan combined with captopril in treating diabetic nephropathy was more effective and rational without any side effect.
