介入性B超在高血压脑出血微创清除术中的应用(附26例报告)

摘　　要：目的总结并探讨介入性B超引导下高血压脑出血微创清除术的治疗经验.方法统计我院神经外科2002年4月～2004年5月在介入性B超引导下行高血压脑出血微创手术26例的临床资料.结果15例术中能清除血肿80%以上;9例能清除血肿70%～∞%;术中发现再出血2例.术后3个月GOS分级:恢复良好18例,中残4例,重残2例,植物状态1例,死亡1例.结论介入性B超引导下高血压脑出血微创清除术具有操作简单易行、穿刺脑内血肿准确性高的特点,能及时发现术中脑组织移位和术中再出血,能有效避免术中误伤正常脑组织,从而提高手术疗效.Objective To discuss the therapeutic effect of B -ultrasound minimally invasive operation in treatment of hypertensive cerebral hemorrhage. Methods 26 patients with hypertensive cerebral hemorrhage treated with minimally invasive operation were analyzed based on the methods and prognosis of operation. Result Removing 80% or more hemorrhage in 15 cases, 70% - 80% hemorrhage in 9 cases, and rehemorrhage 2 cases during operation. The curative effectiveness was assessed according to GOS scores after 3 months： 18 recovered well, 4 moderately disabled, 2 severely disabled, 1 was under vegetative state and 1 died. Conclusion Application of B-guided minimally invasive operation in treatment of hypertensive cerebral hemorrhage is feasible. Which can help to find transposition of brain tissue and rehemorrhage during operation in time, and to improve prognosis of operation.
