血清6-磷酸葡萄糖异构酶水平对类风湿关节炎的诊断价值

摘　　要：目的:探讨类风湿关节炎(RA)患者血清6-磷酸葡萄糖异构酶(GPI)含量与其他体液免疫指示之间的相关性及对RA的诊断价值。方法:用ELISA法检测60例RA患者、55例SLE患者、20例其他自身免疫病患者及130例非自身免疫病对照组的血清GPI浓度。用全自动特种蛋白仪检测60例RA患者的体液免疫指标。结果:当GPI以0.33μg/ml为临界时,60例RA患者GPI阳性率为38%,55例SLE患者为29%,20例其他自身免疫病患者为25%,70名正常对照阳性率为4.2%,20例炎症患者为10%,20例肿瘤患者及20例乙型肝炎患者阳性率均为零。当临界值为2.0μg/ml时,60例RA患者GPI阳性率为25%,此时其敏感度为25%,特异度为98.2%;55例SLE患者阳性率为5.5%;20例其他自身免疫病患者、70名正常对照、20例炎症、20例肿瘤患者及20例乙型肝炎患者GPI值均为零。RA患者中GPI浓度与类风湿因子(RF)浓度呈正相关(r=0.828,P<0.05),但与其他体液免疫指标无相关性。结论:GPI对RA的发生可能起到一定的作用。尤其当值>2.0μg/ml时,对RA的诊断具有较高的特异性。Objective To determine the diagnostic value of the serum concertration of glucose-6-phosphate isomerase（GPI） in patient with rheumatoid arthritis （RA） and its correlation with other diagnostic markers related to human humoral immunity. Methods ELISA was used to measure the serum concentration of GPI in patients with RA or systemic lupus erythematosus （SLE） or other autoimmune diseases and the results were compared with other patients without autoimmune diseases. Other immune markers were detected by BNProSpec. Results When the cut off value was set at 0.33μg/ml, the positive rates in 60 RA patients and 55 SLE patients were 38% and 29% respectively. There were also 10% positive in 20 patients with inflammatory diseases. Other 40 samples,20 from tumor patients and 20 from B hepatitis patients, were all negative in results; When the cut off value rose to 2.0 μg/ml, the positive proportion for RA patients was 25%. Its sensitivity was 25% and specificity reached 98.2%. Only 5.5% of the 55 SLE patients showed positive results and all other samples were negative. Meanwhile, the concentrations of GPI were positive in patients with rheumatoid factor but showed no correlation with other markers （r=0.828, P〈0.05）. Conclusions Varied GPI may play a certain role in the pathogenesis of RA. Measuring the serum concentrations of GPI shows remarkable specificity in diagnosing RA when the cut off value is over 2.0μg/ml.
